The undersigned here by grants permission for all aboved named you to participate in any and all activites of the WYLAM YOUTH FOOTBALL ASSOCIATION (WYLAM GATORS).

-I understand that WYFA has a $75 participant fee that incudes team spirit packs,team helmet paint/decals,Banquet ,Trophyand etc.Must finish season to attend banquet and to recieve trophy. NO REFUND

-I assume all risk incidentals of such participation, including transportation to and from all activities.

-I hereby waive,release,absolve, indemnify and agree to harmless WYFA, BYSL, its League Officials, coaches,other players,parents/guardians,volunteers or any persons transporting your child or children.

-I grant permission for emergency first aid to be given to the minor in case of injury.

-1 grant permission for this minor to be taken to emergency room of a nearby hospital if injury is very serious.

-I understan that my child's team picture my be posted on website or programs page and i give permission to have my child indivisual picture posted as well.

-i will be resposible for any equipment the player has recieved during the season as a loaner and agree to return all equiptment at the end of the season or i will be charged the full amount of the purchased cost of equipment.

-I will furnish a certified state birth certificate of the name of participant when requested by the league.

-I also certify my that my child has NO HEALTH OR PHYSICAL IMPAIthere areRMENTS which will hender His/Her abilty to perform as a football player/cheerleader or cause his/her participation to be deemed unsafe to themselves or others. If there are any health/physical problems the WYFA should be aware of please indicate below.
