Locations: First Presbyterian Church of Sturgis, Mi and along Fawn River.

In this Agreement, the terms “I”, “Me”, “My”, or “Mine” refer to the participant listed above and, if the participant is a minor, to the participant's parents or guardians.  I acknowledge that participation in the activity involves risk to the participant and may result in injury of various kinds, including, but not limited to: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.

 

In consideration for the opportunity to participate in the 2026 Mid High Winter Retreat, I acknowledge and accept the risks of injury associated with participation in and travel to and from the activity. I accept financial responsibility for any injury or other loss sustained during the activity or during travel to or from the activity, and for any medical treatment rendered to me that is authorized by the sponsor or its agents, employees, volunteers,  or representatives (collectively referred to as the” Activity Sponsor"). Furthermore, I release and promise to indemnify, defend, and hold harmless the Activity Sponsor from any liability or injury arising directly or indirectly from my participation in the activity, even if such injury arises out of the negligence of the Activity Sponsor.

 

If any dispute related to this agreement or any claim for damages arises, I agree to work in good faith to resolve the matter through a mutually acceptable alternative dispute resolution process.  If the Activity Sponsor and I cannot agree upon such a process, then the dispute will be submitted to an arbitration in accordance with the then-current rules of the American Arbitration Association."

"I grant to Lake Michigan Presbytery the right to secure and store visual and audio media of the above-named youth. I agree that Lake Michigan Presbytery may use such media with or without the youth's name for any lawful purpose including, for example, such purposes as publicity, illustration, advertising, and web content. I have read and understand the above."

"I grant to Lake Michigan Presbytery Mid High Retreat Advisors, adhering to Presbytery's Policy, the right to communicate with the above-named youth via communications tools and applications including SMS/MMS, instant messaging, chat functions, email, group telecommunications or similar technologies. I understand that the above-named child's contact information may become visible and available to other persons (youth and adults) participating in the communication who may gain access to said technologies."
