Welcome to the Missoula Area Youth Hockey Association (MAYHA) financial assistance application. Thanks to the incredible generosity of local community donors, we are proud to offer a financial aid program designed to help make youth hockey accessible for all local players and their families.
If your family needs support to get on the ice this season, please complete this confidential online form to get started.
Applications must be filled out completely and include all required documentation. Incomplete applications will not be accepted. We accept applications on a rolling basis throughout the year. To ensure your assistance is secured before a program begins, please submit this form by the registration deadline for that specific program.
Winter house and travel players must register online through the Glacier Ice Rink's online registration system and should select the payment plan option at checkout. If financial assistance is awarded, the applicant’s account will be credited with the awarded amount.
Note for Learn to Play and Fall/Spring Programs: If your player is participating in Learn to Play or our Fall/Spring programs, you may submit this application before completing program registration. Please select the appropriate program(s) on the form below (Learn to Play, Fall/Spring, or House/Travel) to ensure your assistance is applied correctly.
Please be sure to fill out this application completely. If you are applying for multiple players in your household, please submit a separate form for each child. Please note that incomplete applications or missing documentation may delay the review process or result in a denial of assistance.
Information:
- To support as many local families as possible, our financial assistance program can cover up to two-thirds (66%) of a player’s registration fees. Families are responsible for the remaining balance of at least one-third of the total cost.
- Financial assistance is strictly allocated toward MAYHA registration fees and equipment rental costs. Please note that these funds cannot be applied to USA Hockey membership fees or any personal travel expenses, including gas, meals, and lodging.
- Financial assistance is awarded on a seasonal basis. Receiving an award for the current year does not guarantee funding for future seasons, and a new application must be submitted each year.
- Financial assistance is awarded at the discretion of the MAYHA review committee, based on the household need demonstrated in the application. To ensure a fair and timely process for all families, all allocation decisions are final.
- You will receive an email update regarding your application status within 14 days of submission. Please keep an eye on your inbox for a message from the Hockey Director.
- All information provided in this application is strictly confidential and will only be used to determine financial assistance eligibility. Please note that all submitted applications and supporting documents become the permanent property of MAYHA.
Application Criteria and Instructions
Financial assistance is open to any current MAYHA player in good standing. To be eligible for review, families must have a clear account balance with no outstanding fees from previous seasons. Awards are determined based on demonstrated financial need and a thorough review of the completed application.
