PEAK GOALKEEPER ACADEMY PARTICIPATION AGREEMENT & LIABILITY WAIVER
In consideration of participation in present and future programs, training sessions, clinics, camps, and events operated by Peak Goalkeeper Academy, I acknowledge and understand that participation in soccer and goalkeeper training activities involves inherent risks of injury. These risks may include, but are not limited to: diving, jumping, collisions with players or equipment, falls, slips on turf or wet surfaces, ball impact injuries, hand and finger injuries, head injuries, concussions, muscle strains, sprains, fractures, heat-related illness, and other injuries that may occur during training, competition, or use of athletic facilities.
I voluntarily assume all risks associated with participation in Peak Goalkeeper Academy programs and use of associated facilities and fields.
I understand and agree that Peak Goalkeeper Academy, its owners, coaches, employees, independent contractors, staff members, facility partners, and agents shall not be held liable for any personal injury, illness, property damage, loss, or other damages arising from participation in any Peak Goalkeeper Academy activity, except where prohibited by law.
On behalf of myself and/or my child, I hereby fully release, discharge, and hold harmless Peak Goalkeeper Academy and all associated facilities, owners, employees, coaches, staff members, and agents from any and all present or future claims, demands, causes of action, liabilities, damages, or expenses arising from participation in Peak Goalkeeper Academy programs or use of related facilities.
I certify that the participant is physically capable of safely participating in soccer and goalkeeper training activities and has no medical condition that would prevent safe participation unless disclosed in writing to Peak Goalkeeper Academy prior to participation.
I agree to follow all rules, instructions, and standards of conduct established by Peak Goalkeeper Academy staff. I understand that unsafe, disruptive, or inappropriate behavior may result in suspension or removal from participation without refund.
In the event of an accident, injury, or medical emergency, I authorize Peak Goalkeeper Academy staff to seek and obtain reasonable medical treatment for the participant if I cannot be reached immediately. I understand that I am responsible for any resulting medical expenses.
I hereby grant permission to Peak Goalkeeper Academy and its representatives to photograph and/or record participants during programs and to use such photographs, videos, or likenesses for promotional, marketing, website, social media, and other business-related purposes without compensation.
I acknowledge that participation in athletic activities may involve exposure to communicable illnesses, including COVID-19 and other viruses or infections, and voluntarily assume such risks associated with participation.
By enrolling in Peak Goalkeeper Academy programs, I acknowledge that I have read, understood, and agreed to the terms of this Participation Agreement & Liability Waiver.
Refund Policy:
All registrations are considered final. Refunds or credits, if any, are issued solely at the discretion of Peak Goalkeeper Academy.
