    All activities, transport, food and drink, are provided free of charge
  • Please complete the form below if you wish your child/young person to: a) take part in any of KPTDT’s Summer of Play Programme of Activities 2026, that will take place at various community venues including, but not limited to Gladstone Hall, Bridgend Community Garden, Three Villages Community Café, trip destinations to Closeburn Castle and Wanlockhead Lead Mining Museum. There will be adults to accompany all participants between venues. No participants will be left unsupervised. Two adult helpers or appropriate participant adult ratio will always be present. And b) be given first aid or urgent medical treatment or for emergency services to be contacted during activity or trip or to attend A&E.   Please note the following information before submitting this form: The activities and events covered by this consent include: • All activities/events to be held in the Gladstone Village Hall • All activities/events to be held in Bridgend Community Garden • Trips to Closeburn Castle Escape Room and Wanlockhead Lead Mining Museum including transport to and from • Walking Bus between venues • Allergens/Dietary requirements • Medical Care • If we need to contact you regarding an emergency/Incident • The use of photographic images for the purpose of further promotion via social media or reporting to funders • Contract of Behaviour and Good Conduct It is the responsibility of the parent/primary care giver to provide the relevant information regarding medical requirements, dietary needs, allergies and provide emergency contact details. Your child/children/young person must be accompanied by at least one adult at the finale Wild West themed communmity disco. Participants will be unable to attend our KPTDT Summer of Play 2026 until all information is completed and signature added below.
