Waiver & Consent

By registering for this event, I hereby grant Dante Moore, Equity Sports, and any partners, affiliates, or organizations associated with the Dante Moore Girls Flag Football Camp on July 19, 2026, the absolute right and permission to take, edit, publish, distribute, and make use of any and all pictures or video taken of my child, to be used in and/or for legal promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites, and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats, and markets now known or hereafter devised and shall continue indefinitely unless revoked in writing by the above-mentioned organizations. I understand and agree that these materials shall become the property of the above-mentioned organizations, partners, and affiliates and will not be returned. I hereby release, discharge, and agree to indemnify and hold harmless all such parties from any and all liability, petitions, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons may bring while acting on my behalf or on behalf of my estate.

I confirm that my child is in good health and has no current conditions that would make it unsafe to participate in athletic activity. The Dante Moore Girls Flag Football Camp will have medical staff present on-site. In the event of an injury or medical emergency, I authorize camp staff to contact emergency medical services on behalf of my child. If I cannot be reached, I hereby consent to any necessary emergency medical treatment, including transportation to the nearest medical facility, being administered to my child by qualified medical personnel. I agree to assume full financial responsibility for any medical expenses incurred.

While participating in events held or sponsored by the Dante Moore Girls Flag Football Camp, its partners, and affiliates, consistent with CDC guidelines, participants are encouraged to practice hand hygiene, social distancing, and wear face coverings if desired to reduce the risks of exposure to COVID-19. Because COVID-19 is extremely contagious and is spread mainly through person-to-person contact, the Dante Moore Girls Flag Football Camp and its partners and affiliates have put in place preventative measures to reduce the spread of COVID-19. However, no guarantee can be made that participants, volunteers, partners, affiliates, or others in attendance will not become infected with COVID-19.

I specifically assume all risks and hazards associated with my child’s participation in this activity, including but not limited to physical injury, accidents, and risks associated with athletic participation, as well as risks associated with COVID-19 or similar viruses. I understand that my child will be associating with staff, volunteers, partners, affiliates, and other participants, and may contract COVID-19 or other viruses through participation. I voluntarily assume the risk that COVID-19 may be transmitted among participants, family members, and household members.

To the fullest extent permitted by law, I hereby agree to waive, release, and discharge any and all claims, causes of action, damages, and rights of any kind — including personal injury, serious injury or death, illness, economic loss, or property damage — against Dante Moore, Equity Sports, the Dante Moore Girls Flag Football Camp, and all partners, affiliates, and sponsors associated with this event, including in each case their respective directors, officers, employees, volunteers, and agents (collectively, the “Released Parties”).

This release applies to claims that I, my heirs, next of kin, and/or legally appointed representatives may have or that may hereafter accrue, arising from or relating in any way to my child’s participation in this event. To the extent permitted by law, I agree to indemnify and hold harmless the Released Parties from any claims, damages, losses, or expenses, including reasonable attorneys’ fees, arising out of or resulting from my child’s participation in this event. This waiver shall be governed by the laws of the State of Michigan.

I agree that my child will conduct themselves in a respectful and sportsmanlike manner throughout the event. The camp reserves the right to remove any participant whose behavior is deemed unsafe, disruptive, or disrespectful. I understand that the Dante Moore Girls Flag Football Camp is not responsible for transportation to or from the event and I assume full responsibility for my child’s transportation arrangements.
