PARTICIPATION WAIVER & RELEASE OF LIABILITY
Mayfield Sportsplex / Mayfield Champions League

1. Acknowledgement of Risk
I acknowledge that participation in the Mayfield Champions League, including all soccer-related activities such as warm-up, gameplay, training, and any associated events, involves inherent risks. These risks include, but are not limited to, physical injury, collision with other participants, contact with equipment or playing surfaces, falls, and other unforeseen hazards. These risks apply to all divisions, including Men’s, Women’s, and Co-Ed play.

2. Assumption of Risk
I understand that participation is voluntary and that I (and my team) choose to participate in the Mayfield Champions League at our own risk. I accept full responsibility for any injury, illness, damage, or loss that may occur as a result of participation.

3. Medical Fitness
I confirm that all registered team members are physically fit to participate in competitive soccer activities and have no medical conditions that would prevent safe participation. Each participant is responsible for ensuring they are medically cleared to play if there is any uncertainty.

4. Team Representation & Collective Agreement
I acknowledge that I am registering and signing this waiver on behalf of my entire team, including all listed players, substitutes, and any additional participants affiliated with the team.
I confirm that I have full authority to bind all team members to the terms of this waiver.
Any individual team member who does not agree to these terms is not permitted to participate in the league.

5. Release of Liability
I hereby release, discharge, and hold harmless Mayfield Sportsplex, Mayfield Champions League, event organizers, staff, officials, referees, volunteers, property owners, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of injury, loss, or damage sustained during participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

6. Code of Conduct – Zero Tolerance Policy
The Mayfield Champions League maintains a strict zero-tolerance policy for violence and misconduct.

Any form of fighting, physical altercation, or violent conduct will result in immediate suspension or permanent ban of the individual and/or entire team at the sole discretion of league officials.
Any verbal abuse, threatening behaviour, harassment, or unsportsmanlike conduct toward players, officials, staff, or spectators may result in disciplinary action, including suspension or removal from the league.
Incidents involving violence or serious misconduct may be reported to law enforcement.
No refunds will be issued to any team or individual removed or suspended due to disciplinary action, including fighting or violent conduct.

7. Compliance with Rules
All participants agree to comply with facility rules, league regulations, and instructions from referees and staff at all times. Failure to comply may result in disciplinary action, including removal from the league without refund.

8. Media Release
I acknowledge and agree that photos and videos may be taken during league activities and may be used by Mayfield Sportsplex and Mayfield Champions League for promotional, marketing, and operational purposes without compensation or further approval.

9. Liability for Team Members
The signing representative accepts responsibility for communicating all league rules, policies, and safety requirements to every member of the team and ensuring compliance throughout the season.

10. Agreement
By registering for or participating in the Mayfield Champions League, I confirm that I have read, understood, and voluntarily agree to all terms and conditions outlined in this waiver on behalf of myself and my entire team.
