2026佛光山達拉斯講堂孝親報恩梁皇法會登記表

法不孤起，仗境方生。梁皇誌公，興慈運悲，

啟殊勝之懺法，建銷罪之功績；

三途親恩，得以脫苦，淨梵人天，蓮品增上

佛光山達拉斯講堂為倡導《孝道月》，發揚孝親報恩、慎終追遠之美德，謹訂於2026年8月27至30日(星期四至星期日)，啟建「孝親報恩梁皇法會」，並於圓滿日舉行三時繫念佛事一堂。以此功德迴向歷代祖先蓮品增上；眾信徒福慧增長；普願社會安和樂利，世界和平。屆時歡迎闔府蒞臨，拈香禮佛，共霑法益。

功德項目:

1.  會        主：US $2000 大牌位6支(消災或超薦)，小牌位20支(消災或超薦)，及一份十供養（不須另登記)

2.  壇        主：US $1000 大牌位4支(消災或超薦)，小牌位10支(消災或超薦)

3.  發   起   人：US $  500 大牌位2支(消災或超薦)，小牌位 6支(消災或超薦)

4.  個人消災祿位：大祿位 US $200，中祿位US $50，小祿位US $20

5.  個人超薦牌位：大牌位 US $200，中牌位US $50，小牌位US $20

6.  其 他 功 德：十供養$120、隨喜贊普、供花果、供齋結緣

 

*登記十供養者請注明是否領取，不領取者由講堂處理。

*如有登記十供養者請於 8月29日梁皇卷十圓滿後，至三樓法堂領取；逾期未領將由講堂處理。  (若更改地點，請留意公告)

In honor of the memory of our ancestors and friends, Fo Guang Shan Dallas Temple will conduct the Emperor Liang Repentance of Filial-Piety & Gratitude Ceremony from August 27 through 30. Prayers will be dedicated towards rebirth in the Pure land for those who have departed, along with peace and blessings for the world we live in. You and your family are cordially invited to participate in this event. 

 

1. Assembly Leader：US$2000, 6 Large plaques, 20 Small plaques, 1 set of Ten Offering (no registration is required)

2. Sponsor：US$1000, 4 Large plaques, 10 Small plaques

3. Co-sponsor：US$500, 2 Large plaques, 6 Small plaques

4. Memorial plaques：Large: $200, Medium $50, Small $20

5. Blessing’s plaques：Large: $200, Medium $50, Small $20

6. Others：Ten Offerings ($120 per set), Flower & Fruit Offering, Meal Offering, Universal Food Offering

*Please indicate if you wish to pick up your offerings at the temple.

*If you register for a set of Ten Offering, you can pick up your Ten Offering after the scroll 10 service on 8/29. Failed to pick up on 8/29 will be handled by the temple.

登記「會主」、「壇主」及「發起人」須知

• 【會主】 : 大牌位6支 (榜文名字兩位)，小牌位20支 。
  （例：登記消災大祿位1支+超薦大牌5支，小消災祿位10支+小超薦10支。您也可以只登記消災或只登記超薦牌位，只要總數不超過6+20） 
• 【壇主】 : 大牌位4支 (榜文名字一位)，小牌位10支 。
  （例：登記消災大祿位1支+超薦大牌3支，小消災祿位5支+小超薦5支。您也可以只登記消災或只登記超薦牌位，只要總數不超過4+10）
• 【發起人】: 大牌位2支(榜文名字一位)，小牌位 6支。
  （例：可以不登記消災大祿位，只登記超薦大牌2支，小消災祿位2支+小超薦4支。可以只登記消災或只登記超薦牌位，只要總數不超過2+6）
• 登記「消災祿位」，只接受登記個人名字或商號，不接受「閤家」。
• 登記「超薦牌位」, 陽上對亡者的稱呼必須一樣, 1~4人名, 需同輩份。只需提供中文或英文一種名字即可。
• 祖先牌位、曾祖/祖父母、父母可以個別登記。如要合拼，請選擇「祖先+長輩」, 並在「亡者姓名」欄寫上X氏與姓名（ 例 ：王氏+王某某 (父) + 李某某 (母）)。
• 登記「地基主」，請將地址輸入在 「亡者姓名」欄，陽上僅限「個人」名字，不接受「閤家」。 
• 登記「累劫冤情債主」及「法界一切眾生」，僅限「個人」登記，不接受「閤家」。

 

Notice for "Assembly Leader", "Sponsor" and "Co-sponsor"

• Assembly Leader, Sponsor and Co-sponsor can register both blessing plaques and/or memorial plaques up to the total quantity. Assembly Leader 6 large plaques + 20 small plaques. Sponsor 4 large plaques + 10 small plaques. Co-sponsor 2 large plaques + 6 small plaques. 
• To register a blessing plaque, we only accept an individual or organization name, not for "family".
• To register a memorial plaque, benefactors of the deceased must be relations from the same generation. Maximum FOUR benefactor names. Please provide either Chinese name(s) or English name(s), not both.
• Plaque should be dedicated to individuals rather than for ancestral clan. However, if you wish to combine plaques of your clan with great grand/ grandparents or parents, you need to select "ancestor + elderly", then write down the following in the deceased name column, eg. "Family's surname (eg. Wong) + XXX (eg.father's name) + XXX (eg. mother's name) ". 
• To register a foundation spirit, please write the address at "Name of the Deceased". Please do not write "XXX family", we only accept individual name(s).
• For registration of "Karmic Creditors" or "All sentient beings in the Ten Dharma Realms", we only accept individual name (s).

 

十供養登記  Ten Offerings to Buddhas & Bodhisattvas 

「十供養」源自於《梁皇寶懺》，又名《慈悲道場懺法》。《梁皇寶懺》共十卷，每卷皆以一種供養，供養諸佛菩薩蒞臨壇場：香、花、燈、塗、果、茶、食、寶、珠、衣，各有其不同的象徵意義與功德。

1. 登記會主者，講堂將代您準備一份十供養，不需另外登記十供養。

2. 登記一份十供養 $120, 梁皇法會第十卷結束後將領取一份十供養，含香、花、燈、塗、果、茶、食、寶、珠、衣。

您可以選擇領取或不領取十供養。不領取的十供養將交由講堂處理。

The Ten Offering originated from the ten scrolls of the Emperor Liang Repentance Service. Each scroll is devoted to one type of offering to the Buddhas and Bodhisattvas, incense, flower, candle, skincare product, fruit, tea, food, treasure, beads, clothes. Each item carries its own significant meaning.

1. If you registered as a "Assembly Leader", you will receive one set of the Ten Offering.

2. If you register for a set of Ten Offering $120, you can pick up your Ten Offering after the service has completed.

You have the option to accept or decline taking Ten Offerings.

贊普 Universal Food Offering to Sentient Beings in the Ten Dharma Realms

三時繫念法會將設贊普台。「贊普」的意思，是透過出家眾誦經、持咒，將供品轉化成甘露法食，以供養四生六道一切眾生，祈願他們心開意解，往生極樂世界。贊普台設於8/30，下午一時三十分舉行三時繫念法會。

* 注意: 此供養以無祀孤魂等眾為供養對象

Universal Food Offering is an offering made to all sentient beings with the ten dharma realms. Through chanting and reciting mantras, every sentient being will be able to receive your gift in aiding their liberation. The offering altar will be set up on 8/30, and a Amitabha Triple Contemplation Service will be conducted between 1:30 - 5:30 pm.

* 收據將依據支票上的名字和地址寄出, 如郵寄地址不一樣, 請說明。
