    Professional Support Groups for Pelvic Floor Providers
  • This Employer Membership Agreement (“Agreement”) outlines the terms and conditions under which your organization* (“Employer”) enrolls one or more employees (“Participant/s”) in professional support groups offered by The Reset Room, LLC (“Organization”).

    1. Purpose of Services
    The Organization provides professionally facilitated support groups for pelvic floor providers. These groups are designed to support Participant self care, professional development, peer consultation, and other intended or unintended benefits to the Participants working in this specialty area.

    2. Nature of Services
    Employer acknowledges that these groups are not psychotherapy, counseling, clinical supervision, or mental health treatment. Participation does not establish a therapist-client relationship between Participants and the Organization or its facilitators. These groups are not a substitute for psychological, psychiatric, or medical care even when facilitated by practitioners of the before mentioned fields.

    3. Participant Eligibility & Responsibility
    Employer affirms that enrolled Participants are appropriate for a professional peer support environment and are licensed or pre-licensed professionals working within the pelvic floor field. Employer agrees that Participants are responsible for:
    Maintaining professional conduct
    Exercising independent clinical judgment
    Seeking appropriate supervision, consultation, or mental health care outside of this group as needed

    4. Confidentiality & Privacy
    The Organization emphasizes the importance of confidentiality within group settings. However, Employer understands and agrees that:
    Confidentiality cannot be guaranteed in a group environment
    The Organization is not liable for breaches of confidentiality by Participants
    Employer will not request or require disclosure of specific content shared by Participants within the group

    5. Non-Evaluative Environment
    These groups are not evaluative in nature. The Organization does not provide performance reviews, employment recommendations, or formal assessments of Participants to Employers even in the event of litigation related to malpractice, employment matters, or otherwise unless required by law. 

    6. Fees & Payment Terms
    Employer agrees to the membership fee structure associated with the selected membership tier. Membership fees are billed on a recurring basis according to the Employer's selected billing schedule.

    If the number of enrolled Participants changes and results in eligibility for a different membership pricing tier, the membership fee will be adjusted to the applicable tier beginning with the next billing cycle. Changes to Participant enrollment during a billing period will not result in prorated charges, credits, or refunds.

    Failure to remit payment may result in suspension or termination of Participant access until the account is brought current.

    7. Scheduling & Attendance
    The Organization will make reasonable efforts to maintain a consistent group schedule. Employer acknowledges that:
    Sessions may occasionally be rescheduled due to facilitator availability or unforeseen circumstances
    Missed sessions are not guaranteed to be rescheduled or refunded unless otherwise specified

    8. Limitation of Liability
    To the fullest extent permitted by law, the Organization shall not be liable for any indirect, incidental, or consequential damages arising from participation in the groups. Employer agrees that Participants engage voluntarily and assume responsibility for their own professional and personal well-being.

    9. Termination
    Either party may terminate this Agreement in accordance with the agreed-upon cancellation policy. The Organization reserves the right to remove a Participant from the group if their behavior is disruptive, unethical, or inconsistent with the group’s purpose.

    10. Cancellation Policy
    Membership may be canceled after completion of the initial three (3) month minimum membership commitment by providing at least thirty (30) days' written notice to the Organization at hello@projectresetroom.com. Membership fees will continue through the 30-day notice period.

    Failure to provide the required 30-day written notice will result in an early termination fee equal to two (2) months of the applicable membership fees. Employer shall be responsible for timely payment of all outstanding fees and authorizes the Organization to take reasonable steps to collect amounts due.

    Changes to the number of enrolled Participants do not constitute cancellation of the membership.

    11. Independent Relationship
    Nothing in this Agreement shall be construed as creating an employment, supervisory, business partnership, joint venture, or agency relationship between the Organization and the Employer or Participants.

    12. Governing Law
    This Agreement shall be governed by and construed in accordance with the laws of the state in which the Organization operates.

    Acknowledgment & Authorization

    By signing below, the Employer acknowledges that they have read, understood, and agree to the terms of this Agreement and authorize the enrollment of designated Participants.

    Upon completion of this form you will receive an invoice for your Company's membership to the email address you provide below. Access to The Reset Room will not be available until your membership is paid & consent forms have been received.
