Participant Waiver, Release of Liability, and Parental Consent
By registering for and participating in this basketball event, I acknowledge that participation in basketball and related activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, physical exertion, and other risks that may result in serious injury, illness, property damage, or death.
I voluntarily assume all risks associated with participation in this event. I understand that participation is voluntary and that participants should only participate if they are physically capable of doing so.
In consideration of being allowed to participate, I, on behalf of myself and/or my minor child, hereby release, waive, and discharge the event organizers, sponsors, volunteers, officials, facility owners, and any affiliated individuals or organizations from any and all liability, claims, demands, actions, causes of action, damages, costs, or expenses arising out of or related to participation in this event, including any injury, illness, loss, or damage that may occur, except as otherwise provided by applicable law.
I understand that the event organizers do not provide medical insurance for participants and that I am solely responsible for any medical expenses incurred as a result of participation.
For Participants Under 18 Years of Age
As the parent or legal guardian of the minor participant named in this registration, I give permission for my child to participate in this event. I acknowledge the risks associated with participation and agree to assume those risks on behalf of my child. I further agree to release and hold harmless the event organizers, sponsors, volunteers, officials, facility owners, and affiliated organizations from any claims arising from my child's participation in the event.
By signing the box below and submitting this registration, I certify that I am at least 18 years old and, if registering a minor, that I am the parent or legal guardian authorized to provide this consent.
