2. Actions/Behaviors that will demand immediate expulsion from camp
- Not being able to be found by staff/veering off from group
- Entering into any other property that is not approved by staff
- Any inapropriate intermingling between opposite gender
- Swimming in lake without chaperone
- Bringing any weapons of any sort
- Not ovbeying to guidelines/instructions given by any staff
- Inappropriate or Immodest clothing
- Leaving campground premises
3. Consent
I hereby attest that I am (we are) the legal parentguardian(s) of the above-named child and hereby consent to the child's participation in the activities described above. I understand that activities of the kind described above may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities.
4. Insurance
I/We represent that the child is (a) covered by insurance through my own insurance carrier; or (b) that I/We am personally financially responsible for any and all medical costs incurred as a result of the child's injury.
5. Emergencies
If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to and authorize Cornerstone Spokane Valley’s Apostolic Church, Cornerstone South and/or Cornerstone North volunteers to make any decision and take any action to arrange for such procedures or treatments in the discretion of the activity supervisor(s).

6. Release of Liability
In consideration of Cornerstone Spokane Valley’s Apostolic Church, Cornerstone South and/or Cornerstone North allowing My Child to participate, I, for myself, my spouse, My Child, and our respective heirs, assigns, and legal representatives, hereby release, waive, and forever discharge Cornerstone Spokane Valley’s Apostolic Church, Cornerstone South and/or Cornerstone North, its pastors, employees, volunteers, agents, and representatives (collectively "Releasees") fromany and all claims, demands, or causes of action arising out of negligence, whether active or passive, related to My Child's participation in the camp.
7. Indemnification and Hold Harmless
I agree to indemnify, defend, and hold harmless Cornerstone Spokane Valley’s Apostolic Church, Cornerstone South and/or Cornerstone North and its Releasees from and against any and all claims, losses, damages, or liabilities—including attorney fees and court costs—brought by anyone (including My Child, co-participants, or third parties) arising from My Child’s conduct, participation in camp activities, or any injuries sustained by My Child during the event.
8. Media
Cornerstone Spokane Valley’s Apostolic Church, Cornerstone South and/or Cornerstone North the absolute and irrevocable right to use, publish, and display photographs, videos, or audio recordings of My Child taken during the camp for promotional materials, websites, and social media without compensation.
