Outdoor Activity Participation and Liability Waiver:
I, the undersigned parent or legal guardian, grant permission for my child to participate in all Vacation Bible School activities sponsored by Restoration Church, including indoor and outdoor activities, games, recreation, field activities, and special events.
I understand that participation in these activities involves certain inherent risks, including but not limited to falls, weather-related conditions, physical activity, and other unforeseen circumstances. I voluntarily assume these risks on behalf of my child.
I agree to release, waive, and hold harmless Restoration Church, its pastors, staff, volunteers, leaders, and representatives from any and all claims, liabilities, damages, costs, or expenses arising from my child's participation in VBS activities, except in cases of gross negligence or willful misconduct.
In the event of an emergency, I authorize Restoration Church personnel to obtain medical treatment for my child if I cannot be reached immediately.
Photo and Video Release:
I grant permission to Restoration Church to photograph, videotape, and record my child during Vacation Bible School activities.
I understand these images and recordings may be used by Restoration Church for:
- Church website
- Social media platforms
- Church publications and newsletters
- Promotional materials
- Presentations and church-related communications
I understand that no compensation will be provided for the use of these photographs or videos.
