Liability Waiver & Acknowledgement of Risk

I, the undersigned parent/guardian of the child/children enrolled in this form, acknowledge and agree to the following:

I understand that swimming and aquatic activities involve inherent risks, including the risk of personal injury.

I consent to my child/children participating in the Holiday Swim Clinic conducted by the Clem Jones Centre.

I confirm that my child/children are medically fit to participate and I will notify staff of any medical conditions, allergies, or special requirements prior to the program commencing.

I agree to follow the directions of swim instructors and staff, and ensure my child/children understand and follow safety rules.

I release and indemnify the Clem Jones Centre, its staff, and representatives from any liability for injury, loss, or damage arising from participation, except where caused by negligence.

I have read, understood, and agree to the above Waiver & Acknowledgement of Risk for the child/children enrolled.

Photo/Video Release

At times during our programs, photos or short videos may be taken for promotional and educational purposes (e.g. social media, website, flyers, or internal training)
