Thank you for your interest in becoming part of Warrior of Faith Motorcycle Ministry. We are a Christ-centered ministry committed to faith, brotherhood, service, and sharing the Gospel of Jesus Christ.

Please carefully review the information below before completing your application.

Because our ministry regularly serves and interacts with vulnerable adults, children, families, and communities, every applicant is required to complete a criminal background check.

A criminal record does not automatically disqualify an applicant. However, offenses involving children or vulnerable adults may require additional review by ministry leadership and the local Chapter President.

By submitting this application, you authorize Warrior of Faith Motorcycle Ministry to conduct a criminal background check as part of the membership review process.

You understand that:

• Background screening may include criminal history and related records permitted by law.

• Screening may be conducted before acceptance into the ministry.

• Additional screenings may be conducted during membership when legally permitted unless authorization is revoked in writing.

• Warrior of Faith LLC may share the results of the background check with the Chapter President associated with your application.

You have the right to request information regarding the nature and scope of any investigative consumer report.

☑ Chapter Commitment: You commit to supporting your local chapter and treating fellow members with respect, humility, honor, loyalty, and integrity.

☑ Biblical Conflict Resolution: You agree to resolve disagreements and conflicts according to Biblical principles and the teachings outlined in Matthew 18.

☑ Ready to Serve: You are prepared to begin your journey as a Warrior of Faith, standing alongside fellow believers in faith, brotherhood, servant leadership, and ministry as together we advance the Kingdom of God.
