Atlas Academy Early Learning Enrollment Form

Atlas Academy Early Learning Center accepts private-pay tuition, School Readiness (ELC), VPK, and other approved scholarship or tuition assistance programs. Please provide any applicable funding information below so we can determine eligibility, coordinate benefits, and accurately calculate your family's tuition responsibility. Families are responsible for any balances not covered by an approved funding source.

I understand that Atlas Academy Early Learning Center may accept payments from third-party funding sources, including but not limited to School Readiness (ELC), Voluntary Prekindergarten (VPK), scholarships, grants, subsidies, and other tuition assistance programs on behalf of my child.

I acknowledge and agree that:

• My child’s enrollment is subject to the rules and requirements of any funding program utilized.
• I am responsible for maintaining eligibility for any funding program, including completing required renewals, submitting documentation, reporting changes in income or household status, and complying with attendance requirements established by the funding agency.
• I am financially responsible for all tuition, fees, and charges not covered by the funding source, including but not limited to registration fees, supply fees, late payment fees, late pick-up fees, returned payment fees, field trip fees, and any other applicable charges.
• If funding is denied, terminated, suspended, reduced, delayed, or expires for any reason, I remain responsible for all tuition and fees incurred during my child's enrollment, including any balances retroactively assessed by the funding agency.
• I understand that funding agencies may require parent co-payments, attendance compliance, or other obligations. Any required parent co-payments are due according to Atlas Academy's payment policies.
• I authorize Atlas Academy to communicate with funding agencies, scholarship organizations, and related service providers as necessary to verify eligibility, enrollment, attendance, and payment information.
• I understand that failure to maintain funding eligibility or pay any outstanding balance may result in collection efforts, suspension of services, or withdrawal from the program in accordance with Atlas Academy policies and applicable laws.
• I agree to promptly notify Atlas Academy of any changes affecting my child's funding status, including changes in eligibility, scholarship awards, family circumstances, or contact information.

I understand that it is my responsibility to update this form in the event that my decision for permission changes. I agree that this form will remain in effect during the term of my child's enrollment.

I, the undersigned, hereby grant Atlas Academy Early Learning Center permission to use photographs of my child named above in any and all of its publications, including website entries without payment or any other consideration. I understand and agree that these materials will become the property of Atlas Academy Early Learning Center and will not be returned.

I hereby irrevocably authorize Atlas Academy Early Learning Center to edit, alter, copy, exhibit, publish or distribute these photos for purposes of publicizing the center's programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy wherein my child's photograph appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.

I hereby hold harmless and release and forever discharge Atlas Academy Early Learning Center from all claims, demands, and causes of action, which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate may have or may have by reason of this authorization.
