Participant Waiver, Release of Liability, and Media Consent
By submitting this registration form, I agree to the following on behalf of myself and/or any minor participant(s) I am registering:
I understand that participation in Swayzee Days activities and events is voluntary and may involve physical activity, the use of equipment, interaction with the public, animals, vehicles, and outdoor conditions. I acknowledge that participation may involve risks including, but not limited to, falls, collisions, weather-related hazards, property damage, illness, injury, or other unforeseen circumstances.
I knowingly and voluntarily assume all risks associated with participation in Swayzee Days activities and accept full responsibility for any injury, illness, loss, damage, or expense that may result from participation.
In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless Swayzee Days, LLC, its directors, officers, volunteers, event coordinators, sponsors, partners, and agents, as well as the Town of Swayzee and its officers, employees, and volunteers, from any and all claims, liabilities, demands, actions, damages, costs, or expenses arising out of or related to participation in Swayzee Days activities, except where prohibited by law.
If I am registering a minor participant, I certify that I am the parent or legal guardian of the child(ren) listed on this registration and grant permission for their participation in the selected activity(ies). I understand and accept the risks associated with their participation and agree to this waiver on their behalf.
I grant permission to Swayzee Days, LLC to photograph, record, and use images, video, and audio of participants for promotional, educational, historical, and marketing purposes, including use on websites, social media platforms, printed materials, and other communications. Participants may request exclusion from media use by notifying event organizers in writing prior to the event.
I agree to follow all event rules, safety requirements, and instructions provided by event organizers and volunteers. I understand that unsafe, disruptive, or inappropriate behavior may result in removal from an activity or event without refund or compensation.
