KAISOKAH MOKO JUMBIES USA IS AN ALL INCULSIVE JUNIOR CARNIVAL BAND (UNLIMITED BEVERAGE,SNACKS,FOOD)

Note: This entry form serves as the basis for your acceptance as a participant in our junior carnival band. A minimum deposit of ($50) is required. DEPOSIT is (non-refundable). The Deposit must accompany this registration form so your costume order can be placed. Full payment Is due by August 28th 2026

*******One registration form per child.

******Band T-shirts are $25. Everyone is required to have on a band T-shirt in order to be within the security rope.(T-shirt will be available at distribution)

******No costume order will be placed if form is not submitted and/or deposit is not made.*
