    This Agreement is made on , Date:   Pick a Date      , between

  •  1. THE RELATIONSHIP

    This is a Shared Housing / Roommate Agreement. You will be participating in an independent living program and is being granted a license to use a specific bed space and shared common areas. This is NOT a traditional landlord-tenant lease.

  • 2. PROGRAM FEES & UTILITIES

  • Non-Refundable Community fee: $300 upon moving in.
    Monthly Program Fee: $
    Due Date: Fees are due on the 1st day of each month, no later then the 5th.
    Late Fees: A fee of $25 will be applied if payment is after the 5th of the month.
    Included Utilities: ( Water, Gas, Electricity, Trash, Wi-Fi) are included in the program fee.

  • 3. ROOM ALLOTMENT

    The Occupant is assigned to: Shared Room. The Provider reserves the right to relocate the Occupant to a different room within the property at any time to ensure the safety and harmony of the house.

  • 4. COMMUNITY STANDARDS & SAFETY

     

    * Tobacco use is not allowed

     * "NO Smoking"  in the House, or in front of the property at all.

     * Zero Tolerance: The use, possession, or distribution of illegal drugs, alcohol or weapons on the property is strictly prohibited and will result in immediate termination of this agreement.

     

    *Violence: Any act of violence or threats towards other residents or the Provider will result in immediate termination of this agreement.

     *Quiet Hours - 10:00pm - 7:00am

  • 5. HOUSE ACCESS & INSPECTIONS

    The Provider and their agents maintain the right to enter common areas (kitchen, living room, hallways) at any time for safety checks, maintenance, and house management. Private rooms will be inspected with a 24-hour notice unless there is an emergency.

  • 6. NO GUEST POLICY

    NO Guests are allowed on the property at any time. This ensures the safety and privacy of all roommates

  • 7. TERMINATION OF AGREEMENT

    -The Occupant may terminate this agreement with a 30 day written notice.

     

    -The Provider may terminate this agreement immediately for violations of the Independent Living / Shared Housing Community rules, violence, or non-payment of program fees.

  • 8.SIGNATURES

    By signing below, the Occupant acknowledges they have read, understood, and agreed to the terms of this Shared Housing Program.
