Please read this document carefully before signing. By signing this document, you are waiving certain legal rights, including the right to sue Harris County Soccer Association, Columbus United FC, their directors, officers, employees, and agents.
1. EXPRESS ASSUMPTION OF RISK
I understand and acknowledge that participation in soccer and related camp activities involves inherent risks, hazards, and dangers that cannot be eliminated, including but not limited to severe bodily injury, illness, permanent disability, paralysis, or death. These risks include, but are not limited to, collisions with other players, objects, or the ground; severe weather conditions; facility defects; and exertion of strenuous physical activity. I expressly and voluntarily assume all risk of personal injury, death, or property damage sustained while participating in the camp, whether caused by the negligence of HCSA, Columbus United FC, or otherwise.
2. RELEASE OF LIABILITY AND COVENANT NOT TO SUE
I hereby release, waive, and forever discharge Harris County Soccer Association and Columbus United FC, along with their respective officers, directors, agents, employees, coaches, volunteers, and affiliates (collectively, the "Released Parties"), from any and all liability, claims, demands, or causes of action arising out of negligence, gross negligence, or any other cause, resulting in personal injury, illness, or property damage during participation in the
camp. I covenant not to sue or make any claim against the Released Parties for damages or other losses sustained as a result of camp participation.
3. MEDICAL AUTHORIZATION AND TREATMENT
In the event of an injury, illness, or medical emergency during participation in the camp, I hereby grant permission to the camp staff, coaches, and medical personnel to administer first aid or authorize emergency medical treatment as deemed necessary. I understand and agree that I am solely responsible for any and all costs, fees, and expenses associated with such medical attention, transportation, or hospitalization.
4. MEDIA AND PHOTOGRAPHIC RELEASE
I hereby grant Harris County Soccer Association and Columbus United FC the absolute and irrevocable right to use, publish, and reproduce photographs, video recordings, or digital images taken of the participant during the camp for promotional, educational, or marketing purposes in any medium, without compensation or further notification.
5. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities (including attorney's fees) brought as a result of my (or the participant's) involvement in the camp, and to reimburse them for any such expenses incurred.

By signing below, I certify that I am at least 18 years of age (or the parent/legal guardian of the
participant), that I have read and fully understand this Waiver and Release of Liability, and that I freely and voluntarily agree to all of its terms.
