Please briefly describe your involvement in parish life and why you want to be an OCIA sponsor.

The Church requires that all those who assume the responsibilities and obligations of sponsorship for the Order of Christian Initiation of Adults practice their religion faithfully and are exemplary in their conduct as members of the Roman Catholic Church.

In accepting this responsibility, I truthfully state the following:

I am at least 16 years of age.

I have received the Sacraments of Baptism, Eucharist, and Confirmation in the Catholic Church.

If married, I am not married outside of the Catholic Church

I am not the parent of the person that I wish to sponsor.

I will make an effort to be present at as many OCIA sessions as possible. Sessions are on Monday nights from 6:30 - 8:00 pm from August through May.

I attend Mass regularly on Sundays and Holy Days of Obligation and receive the Sacrament of the Holy Eucharist and Penance regularly.

I understand and accept the responsibility which I undertake as a sponsor and I promise to be an example that reflects the spirit and teaching of the Roman Catholic Church. I am prepared to assist the Candidate by my support, encouragement, and prayer.

I affirm that I fully meet the above eligibility requirements and wish to be an OCIA sponsor.

To ensure that our OCIA sponsors are well-formed, we ask that you set up a time to meet with our Coordinator of Religious Education. Please list a few days and times that you would be available for a 30 minute meeting in the coming weeks. Our Coordinator of Religious Education will reach out to you to confirm a meeting time.

I understand that completion of this form in no way confirms that I will be a sponsor, but merely notifies the DRE of my interest, who will prayerfully consider if I am a good fit for this role.
