Gilliam Brothers Football Camp — Parent/Guardian Waiver

SECTION 1 — WAIVER AND RELEASE OF LIABILITY

In consideration of my child's participation in Gilliam Brothers Football Camp ("Camp"), I, the undersigned parent or legal guardian, hereby voluntarily waive, release, discharge, and hold harmless Gilliam Brothers Football Camp, its directors, coaches, staff, volunteers, and affiliated organizations (collectively "Released Parties") from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by my child while participating in camp activities, whether caused by the negligence of the Released Parties or otherwise. I acknowledge that participation in football activities involves inherent risks, including but not limited to physical contact, sprains, fractures, concussions, and other injuries, and I voluntarily assume all such risks on behalf of my child.

SECTION 2 — MEDIA RELEASE

I hereby grant Gilliam Brothers Football Camp and its authorized representatives permission to photograph, film, videotape, or otherwise record my child during camp activities. I authorize the Camp to use these images and recordings for promotional, educational, and informational purposes including social media, website, print materials, and advertising, without compensation or further consent.

SECTION 3 - MEDICAL CONSENT. I authorize Gilliam Brothers Football Camp staff to seek emergency medical treatment for my child if I cannot be reached. I certify my child is physically capable of participating. I will notify staff of any medical conditions and accept responsibility for medical expenses.

ELECTRONIC SIGNATURE AND ACKNOWLEDGEMENT

By signing below, I confirm that I am the parent or legal guardian of the above-named child, that I have read and fully understand this waiver in its entirety, and that I voluntarily agree to all terms above.
