    Before continuing with this form, there are a couple of key items to review.

    1. If you have multiple athletes, we need a different form for each athlete unless you are having them photographed together and purchasing their package together.  After completing this form, you will have the option to start again with the next athlete.

    2. You can choose from Build Your Own, Package 1, or Package 2 for multiple athletes.

    3. Standard packages are for one pose.  If you are interested in multiple poses, sibling photos or other group photos, take a look at the build-your-own or add-on options.

    3. To expand each package option or to make a purchase, please check the box to the left of the package.
