    Agreement: I, the undersigned player, acknowledge and agree that participation in the Little Haiti Maniacs Adult League Football involves inherent risks of physical injury. I assume full responsibility for any risks, injuries, or damages which may occur during my participation. I hereby release, waive, discharge, and covenant not to sue the Little Haiti Maniacs, its organizers, sponsors, and agents from any and all liability for any loss, damage, or injury to my person or property, regardless of the cause. I also grant the Little Haiti Maniacs permission to use photographs and videos taken of me during league activities for promotional purposes. I certify that I possess adequate health insurance to cover any injury or damage I may cause or suffer while participating, and I agree to bear the costs of such injury or damage myself.

  • Code of Conduct Acknowledgment

    Purpose: The Little Haiti Maniacs (LHM) is committed to providing a safe, enjoyable, and competitive environment for all participants. This Code of Conduct outlines the expected standards of behavior for all players, coaches, and spectators associated with the league.By signing below, I acknowledge that I have read, understand, and agree to adhere to the following principles throughout my participation in the LHM Adult League Football:1. Sportsmanship and Respect• I will treat all opposing players, coaches, officials, and league staff with courtesy and respect at all times, regardless of the game's outcome.• I will refrain from any form of trash talk, baiting, or mockery directed at anyone on or off the field.• I will conduct myself as a role model, demonstrating fair play and integrity.2. Zero Tolerance for Violence and Aggression• I will not engage in any form of physical violence, fighting, intimidation, or aggressive behavior. This includes, but is not limited to, hitting, shoving, kicking, or threatening others.• I understand that any physical altercation will result in immediate expulsion from the game and potential permanent removal from the league.3. Conduct Towards Officials• I will accept the decisions of referees and officials without aggressive protest or dissent.• I will communicate concerns through my team captain in a respectful and non-confrontational manner. Verbal abuse of an official will not be tolerated.4. Substance Abuse• I will not consume or be under the influence of alcohol or illegal drugs before or during any LHM game or official league event.• The use of tobacco products on or near the playing field is prohibited.5. Compliance and Penalties• I agree to abide by all official LHM League Rules and Bylaws.• I understand that the LHM League Directors have the final authority to interpret and enforce this Code of Conduct.• I acknowledge that violation of this Code may result in disciplinary action, including:• Verbal Warning• Game Ejection• Single or Multiple Game Suspension• Permanent Expulsion from the League (without refund)Player Acknowledgment"I certify that I have read the Little Haiti Maniacs Adult League Football Code of Conduct and agree to comply with all terms and conditions outlined above."
