To nominate an individual or organisation, the nominator must:  

1. Ensure that the candidate meets the criteria for The Governor’s Chaplaincy Awards and is able to display evidence of a commitment to the ministry of chaplaincy
   
   
2. Provide a profile of the individual or organisation that establishes grounds for the nomination.
   
   
3. Provide written responses to the specific award category criteria (in less than 200 words)
