Dancers age 3 - 18 years old can schedule a FREE trial class. As soon as you submit this form, the Studio Manager will contact you to set up an exciting trial class!

IDT Participation Waiver

I hereby give permission for the above student to participate in Intensity Dance Team activities both inside and outside of the studio (performances/online classes/video classes). I also assume all responsibility for injuries or illnesses that may occur through the duration of participation in any IDT activities. While acknowledging that precautions will be taken to provide a safe environment, students should be aware that there is a risk of injury due to the inherent nature of the activity.
