Thank you for your interest in completing a Catholic Religious Recognition program! Please complete the form below to declare your intent.

Upon submission of this form, you will receive an email from the Catholic Committee for Girl Scouts of Western Ohio (CCGSWO) confirming your program registration. This email will also contain a personalized link to your Program Completion Form (PCF). If you do not see the email in a few minutes, check your junk mail or spam folder.

You will need to submit the PCF in order to be approved to order your girls’ religious medals. The PCF is submitted with the original Application Pages from each girl’s book, when your girls have completed the required activities for their program. Keep this personalized link handy (file it where you can easily access it) as it is how the CCGSWO identifies your troop and the medal(s) they are working toward.
