I, the undersigned, acknowledge that my participation in The H.E.R.E. Center’s (THC) and True Self Holistic Therapy (TSHT) therapeutic camping trip involves inherent risks, including, but not limited to, physical injury, accidents, and illness. I understand that these risks may arise from my actions, the actions of others, or the conditions of the facilities and environment. In consideration of being allowed to participate in this event, I hereby waive, release, and discharge THC and TSHT, its directors, officers, employees, volunteers, and agents from any and all claims, demands, or causes of action that I may have for injuries, damages, or losses arising out of my participation in the therapeutic camping trip. I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the state in which the event is conducted, and if any portion of this waiver is held to be invalid, the remainder shall continue in full force and effect. I have read this waiver and release agreement, fully understand its terms, and acknowledge that I am signing this waiver freely and voluntarily, intending my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
I, the undersigned, hereby grant permission to THC and TSHT to take photographs and videos of me during the therapeutic camping trip. I understand that these images and videos may be used for promotional purposes, including but not limited to social media, brochures, and other marketing materials. I acknowledge that my participation in this event implies my consent to the use of my likeness in these materials without compensation. I release THC and TSHT, its employees, agents, and representatives from any claims, demands, or liabilities that may arise from the use of these photographs and videos. I understand that I can withdraw my consent at any time by providing written notice to THC and TSHT, but that withdrawal will not affect any uses made prior to the receipt of such notice.
