If you are 16 or 17 years old you must fill out and notarize a Florida State minor tattoo form. They can be found online and at the shop. They must be notarized the same day as your tattoo appointment. A legal parent/ guardian must accompany you to your appointment and must be present during the tattoo. This is a state requirement! :)
