Liability Waiver

Assumption of Risk and Liability Waiver I understand that participation in Schnack Camps activities involves inherent risks of injury. Activities may include, but are not limited to, baseball, basketball, football (including flag football), soccer, golf, speed and agility training, conditioning exercises, warm-ups, stretching, drills, scrimmages, games, competitions, recreational activities, and other sports-related events. I acknowledge that participation may involve risks including, but not limited to, falls, collisions, contact with equipment or other participants, muscle strains, sprains, fractures, concussions, heat-related illness, and other injuries ranging from minor to severe. By allowing the participant to take part in Schnack Camps activities, I voluntarily assume these risks. To the fullest extent permitted by law, I release and hold harmless Schnack Camps, its officers, directors, volunteers, coaches, instructors, staff members, sponsors, facility owners, and affiliated partners from claims or liability arising out of participation in camp activities, except where prohibited by law or resulting from willful misconduct. I understand that Schnack Camps does not provide medical insurance for participants and that I am responsible for any medical expenses incurred. If emergency medical treatment becomes necessary and I cannot be reached promptly, I authorize Schnack Camps personnel to seek appropriate emergency medical care for the participant. I certify that the participant is physically able to participate in athletic activities and that any known medical conditions or limitations have been disclosed.
