Ordering Instructions:
- This order form is for ordering seasonal pre-order cookies only. If you have a custom order, please reach out to me via one of the above methods.
- Placing your order here does NOT confirm it. You will be contacted to confirm order details, order total (with discounts applied), instructions for deposit payment within 48 hours of placing the order.
- Pick-up will be from Livonia, MI 48154 (Five Mile and Middlebelt). If you are unable to pick up during any of the listed times or need the order delivered ($5 delivery charge within 20 miles from 48154), please explain in the comments section at the bottom. While special requests cannot be guaranteed, I will try to accommodate to the best of our ability.

- I understand that 50% deposit is required to confirm the order and the balance is due at pickup/delivery. Methods of payment: Zelle (313-588-1917), Venmo (@DulminiBarupala) or CashApp ($BakedbyMini). Cash can be accepted for the balance at pickup.
- I understand that "Baked by Mini" operates under Michigan Cottage Law and products are made in a home kitchen that has not been inspected by the Michigan Department of Agriculture and Rural Development.
- I understand that "Baked by Mini" home kitchen is not an allergen-free facility and does not operate under allergen free conditions. Decorated sugar cookies contain these allergens at the least: Eggs, Wheat, Milk.
- I understand that cookies are uniquely handcrafted and may vary slightly in shape, size, color, and decorative details from cookies shown.
