This form needs to be filled out so access codes can be input by the AOAO for Waikele Storage Park.

Note: Both codes can only be five (5) digits long. Please do not create them in a sequence, they should be random. if it is the tenants responsibility to control who the access codes are given out to. Please print all information requested legibly and return form to Security Post or Email to property management. (See Below)

It is the responsibility of the tenant to request a Security Access Code change in the event of an employee termination.
