Agreement: As consideration for being allowed to participate and/or compete in the Fortuna Rodeo of the Fortuna Rodeo Association, the particpant listed above agrees to the following:
Acknowledgement of Risk: The Undersigned acknowledges that the Fortuna Rodeo is a dangerous activity and that the participation in a rodeo, as either a
contestant, an employee or volunteer, exposes the Participant to a substantial and serious risk appropriate damage, personal injury, or death. The Undersigned expressly acknowledges that his/her participation in the Fortuna Rodeo will involve such a hazard. The undersigned also agrees Participant will wear proper safety equipment in order to participate in event. No exceptions.
Release of Sponsor: The Undersigned, being fully aware that participation in the Fortuna Rodeo will expose him/her to a substantial and serious risk of property
damage and/or personal injury or death, hereby releases all Sponsors from liability for and all property damage, personal injuries or other claims arising from the Undersigned’s participation in the Fortuna Rodeo including those better known and unknown, foreseen and unforeseen, future or contingent.
Covenant Not to Sue: The Undersigned covenants that the Undersigned shall not now or at any time in the future, directly or indirectly, commence or prosecute any action, suit or other proceedings against the Fortuna Rodeo Association, the
Sponsors (their officers, directors, employees, agents, or affiliates) concerning, arising out of, or related to the actions, causes of action, claims and demands hereby waived, released, or discharged by the Undersigned.
Assurances: The Undersigned has full power, authority, capacity and right without
limitation to execute, deliver and perform this Release.
Binding Affect: This Release shall be binding upon the Undersigned and the Undersigned’s guardian, spouse, legal representatives, heirs, successor’s, and assigns.
Photo Authorization: The Undersigned grants permission to the Fortuna Rodeo Association and/or its affiliates to use all video, photo, and audio recordings from Undersigned’s participation in the Fortuna Rodeo for any
purpose, including television commercial promotions, marketing images on print or digital documents, and for billboards or outdoor or indoor displays. Undersigned also releases the Fortuna Rodeo Association from any claim that may arise from the use of your likeness, including, but not limited to, claims
of defamation to reputation, invasion of privacy, rights of publicity or copyright.
This release has been carefully and fully read by the Undersigned and the Undersigned fully understands its terms and conditions and has voluntarily e-signed below.
