  • RELEASE, WAIVER & INDEMNITY AGREEMENT

  • In consideration of my child's participation in the Summer Sacred Art Children's Camp hosted by the Cathedral of Saint Paul (239 Selby Avenue, St. Paul, MN), I, the undersigned parent or legal guardian, hereby agree to the following:

    As the parent and/or legal guardian of the participant named in this registration, I acknowledge that I remain legally responsible for my child’s personal actions during the Summer Sacred Art Children’s Camp.

    I agree on behalf of myself, my child named herein, and our heirs, successors, and assigns, to hold harmless and defend the Cathedral of Saint Paul, its officers, directors, employees, and agents, and the Archdiocese of Saint Paul and Minneapolis, its employees, agents, chaperones, and representatives associated with this event, from any claim arising from or in connection with my child attending this event, or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith.

    I further agree to compensate the Cathedral of Saint Paul, its officers, directors, and agents, and the Archdiocese of Saint Paul and Minneapolis, its employees, agents, chaperones, and representatives associated with this event, for reasonable attorney’s fees and expenses which may be incurred in any action brought against them as a result of such injury or damage — unless such claim arises from the negligence of the Cathedral of Saint Paul or the Archdiocese of Saint Paul and Minneapolis.
