    There are no right or wrong answers. The more details you share, the better I can help your family. This form usually takes 15 to 20 minutes to finish. Please answer as the adult caregiver. If you live with more than one dog, fill out a separate form for each one.

    This service is for dog caregivers who want help with their dogs’ well-being and behavior. All dog caregivers are welcome, no matter your dog’s age, breed, or background. Your answers are confidential. If a question asks about something you feel is unsafe, please do not try to recreate it. Just describe what you have already seen.
