• Low Cost TNVR Appointment Form

    This form is for the LVHS's Low Cost TNVR days. This day is to assist residents outside our contracted areas with affordable TNVR.

  • ALL CATS MUST BE IN A TRAP, CATS WILL NOT BE ACCEPTED IN ANY OTHER CONTAINER. ONLY ONE CAT PER TRAP WILL BE ACCEPTED.

    ALL TRAPS MUST BE MARKED WITH THE NAME AND PHONE NUMBER OF THE TRAP OWNER.

     A SEPARATE APPOINTMENT IS REQUIRED FOR EACH CAT.

  • NO TREATMENT OR PROCEDURE RECORDS ARE PROVIDED FOR FERAL/COMMUNITY CATS. NO OTHER MEDICAL REQUESTS WILL BE HONORED. THIS APPOINTMENT IS FOR SPAY/NEUTER ONLY.

    I have just legally relinquished a feral/community cat to the Lehigh Valley Humane Society and Vet Clinic.  My relinquishment gives the LVHS legal claim to this feral/community cat while being spayed/neutered.  I am taking steps to help control the community cat population by taking community cats to be spayed/neutered.

     

    The LVHS has agreed to spay/neuter this feral cat and return them to me at which time I will once again assume responsibility for the cat.

     

    I understand that by agreeing with LVHS feral cat spay/neuter program I am relinquishing all ownership rights to this animal until I reclaim the animal from recovery. I understand that the animal(s) I give up may be humanely euthanized if it is deemed unfit to return to field by the LVHS veterinary team or if it is determined to be suffering from a life threatening illness or injury. The Lehigh Valley Humane Society can make no guarantee as to the health of any feral cats being dropped off for spay/neuter procedures. I further understand there are inherent risks associated with any surgery and all medical decisions are made by LVHS medical team. 

    I have not taken this animal from another person without that person’s consent.

    I assume all responsibility for the subsequent placement for all animals returned to me.  I am not seeking medical care for this animal in order for LVHS to assume responsibility for long-term care or for adoption purposes. 

    The Lehigh Valley Humane Society makes no guarantee regarding the overall health and long-term well-being of any feral/community cat once it is released from our care. 

    All TNR cats receive the following treatments/procedures ONLY.

                  Spay/Neuter

                  Left Ear Tip

                  Rabies and Feline HCP Vaccines

                  Topical Flea and Tick Treatment

                  Ear Cleaning

  • The TNVR fee will be paid at the time of drop off. The fee for TNVR is $45.

    This fee can be paid in either cash or card. We do not accept American Express.
