Assumption of Risk and Release of Liability
I understand that participation in the YOUniversal Fitness Studio Reading & Recess Summer Camp may involve physical activities including, but not limited to, running, games, playground activities, fitness exercises, stretching, and recreational activities. I acknowledge that these activities carry inherent risks of injury.

On behalf of myself and my child, I voluntarily assume all risks associated with participation in the camp.

To the fullest extent permitted by law, I release and hold harmless YOUniversal Fitness Studio, Fitness T.R.A.P. LLC, Oasis of Hope Center, their owners, employees, volunteers, contractors, and agents from any claims, demands, damages, liabilities, or causes of action arising from participation in camp activities, except where caused by gross negligence or willful misconduct.

Medical Authorization
In the event of an emergency, I authorize YOUniversal Fitness Studio staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am financially responsible for any medical care provided.

Behavior Expectations

I understand that campers are expected to:

Respect staff, volunteers, and fellow campers.
Follow safety instructions.
Refrain from bullying, fighting, or disruptive behavior.
Respect camp property and equipment.
Repeated unsafe or disruptive behavior may result in dismissal from camp without refund.

Photo & Media Release
I give permission for my child to be photographed or recorded during camp activities. I understand these images may be used by YOUniversal Fitness Studio for marketing, social media, or promotional purposes without compensation.

Refund Policy Acknowledgment
I understand that camp fees reserve my child's space and that refunds may not be available once camp has begun, except as required by law or at the sole discretion of YOUniversal Fitness Studio.

This waiver is a general teplate and should be reviewed by a Washington attorney to ensure compliance with state law and your specific camp operations.
