Welcome to Fall Programming at After-School All-Stars!

We are excited to kick off another amazing season of learning, growth, and fun. By now, we have a good idea of what our students enjoy, what sparks their curiosity, and what gets them excited to participate. Fall programming is the perfect opportunity to build on those interests, explore new experiences, and ignite a passion for learning, leadership, and personal growth.

This season, students can look forward to engaging activities, hands-on projects, academic support, enrichment opportunities, and meaningful connections with peers and mentors. Whether they're discovering a new hobby, developing important life skills, or strengthening their confidence, Fall Programming is designed to help every student thrive.

AFTER-SCHOOL ALL-STARS

MEDIA RELEASE AND CONSENT FORM (MINORS AND ADULTS)
This Media Release and Consent Form applies to both adult participants and minor participants. For participants under the age of 18, this form must be completed and signed by a parent or legal guardian.

I hereby grant full permission and all rights to After-School All-Stars ("ASAS"), its affiliates, agents, representatives, successors, and assigns to use, adapt, edit, reproduce, distribute, publish, display, and otherwise utilize my or my child's:

• Name
• Photograph(s)
• Video recordings
• Voice
• Face
• Likeness
• Background information
• Artwork, written work, or other works produced

obtained through photographic, video, digital, audio, virtual communication platforms, or any other recording methods during ASAS programs, activities, meetings, events, field trips, and related activities, whether conducted in person or virtually.

I understand and agree that these materials may be used in videos, publications, websites, social media, promotional materials, marketing campaigns, reports, fundraising materials, motion pictures, recordings, presentations, and all other current or future forms of media, in perpetuity and throughout the world. I further grant ASAS the unrestricted right to reproduce, copy, exhibit, publish, distribute, broadcast, modify, or otherwise use such materials without limitation.

For participants under the age of 18, to the extent that any image, recording, or media constitutes an educational record containing personally identifiable information as defined by the Family Educational Rights and Privacy Act of 1974 ("FERPA"), I hereby consent to the disclosure and use of such information as described in this release.

I waive any right to inspect, review, or approve any finished product, publication, or media created using the materials described above. I further waive any claim regarding the use of such materials or the purposes for which they may be used.

I understand and agree that neither I nor my child will receive compensation, payment, royalties, fees, or any other remuneration now or in the future for participation in the recording or for ASAS's use of the materials described above. Participation was voluntary and without expectation of compensation.

I hereby waive, release, and discharge any and all rights, claims, demands, causes of action, damages, or liabilities that I, my child, my heirs, representatives, or any third party may have against ASAS and its affiliates, agents, subsidiaries, successors, and assigns arising out of or related to the creation, use, reproduction, adaptation, distribution, publication, broadcast, display, or other use of the materials described above. This includes, but is not limited to, claims for:

• Invasion of privacy
• Right of publicity
• Copyright infringement
• Defamation
• Misappropriation
• Emotional distress
• Any other legal or equitable claim

This release includes any claims arising from editing, alteration, blurring, distortion, optical illusion, composite use, or any other modification, whether intentional or otherwise, including use on the internet, social media platforms, websites, brochures, posters, advertisements, or other promotional materials.

I understand that ASAS cannot control and is not responsible for any unauthorized use, reproduction, sharing, or distribution of images or recordings by third parties who may access such materials through the internet or other means.

I agree to indemnify, defend, and hold harmless ASAS and its affiliates, agents, subsidiaries, successors, and assigns from and against any claims, demands, liabilities, damages, costs, or expenses arising from or related to this Media Release.

I understand that participation in photography, videography, and recording activities is voluntary. I have the right to decline consent for myself or my child and understand that choosing not to provide consent will not affect eligibility to participate in ASAS programs or activities.

This consent shall remain in effect unless and until revoked in writing and delivered to ASAS. Any revocation will apply only to future uses and will not affect any use, publication, or disclosure made prior to ASAS's receipt of the written revocation.

I certify that I have read and fully understand this Media Release and Consent Form. I affirm that the information provided is accurate and that I have the legal authority to execute this release.

Dear Parents/Guardians
After School All Stars is funded by the Office of Out of School Time Grants and Youth Outcomes (OST Office), a DC Government agency. As a grantee, we are required to share participant information with the OST Office that may be collected on the enrollment form such as full name, date of birth, school name, demographics, and age.

In addition, we are required to administer a Youth Survey questionnaire. The Youth Survey is a brief survey with questions about what your child thinks of the program and of the potential benefits of attending the program. All information collected through the Youth Survey is confidential and no individual child or their individual responses will be identified. Participation in the Youth Survey is voluntary.

By signing below I give permission for the student listed below to participate in the Youth Survey.

ASAS has received grant funds from the DC government and US Department of Education to run the afterschool program that your child participates in. As part of these grants, we are required to have an external evaluator to help ASAS understand how the program operates and ways in which the program could be improved. ASAS has contracted with Urban Circle Consulting LLC (UCC) to conduct the evaluation.

We ask your permission for UCC/ASAS staff to administer a survey to your child’s school-day teacher that asks the teacher to report on your child’s progress in academics and classroom behavior over the school year.

We also ask your permission for UCC/ASAS staff to talk to your child about his/her experience in the program as part of a 30-minute focus group of participating students in the program at your child’s school.

Any information collected will be used only to evaluate the ASAS program. Participating in the evaluation will not affect your child in school, in ASAS programs, or in any other way. We will not use your name or your child's name in any report. Participation in the evaluation is completely voluntary, and your child may withdraw at any time with no consequences.  

Parents please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c)(1)(A), you have the right to review a copy of the questions asked of or materials that will be used with your students. Statement of  ASAS Evaluation

After-School All-Stars engages in ongoing program evaluation to identify personal, social, and academic outcomes for students that participate in our programs. Evaluation results are used for continuous improvement of our offerings and for reporting our impact to funders and other stakeholders. During the course of the program, ASAS will collect data about your student’s participation in ASAS activities and surveys about the program experience and student outcomes. Additionally, and ONLY IF YOU CONSENT TO THE FERPA RELEASE OF ACADEMIC DATA BELOW, ASAS may also collect your student’s education records from their district to evaluate academic and school-day outcomes that may result from participation in the program. All data collected is confidential and protected with industry-standard practices. The only individuals who will have access to your student’s outcome or academic data are approved staff or evaluation consultants bound to confidentiality and protection of data privacy. Your student’s data will NEVER be reported individually or in such a way that your student could be identified. All reporting will be in aggregate. If you have any questions about ASAS evaluation or would like to opt your student out of the evaluation, please email evaluation@afterschoolallstars.org.

FERPA Release of Academic Data

Dear Parents/Guardians,

In an effort to serve your child better in our afterschool program, and to ensure that the program meets your child's academic needs, After School All Stars (ASAS) would like to receive copies of certain educational records related to your child that would help us to tailor the afterschool program to meet your child’s needs, further cultivate his/her strengths and identify and develop areas where s/he is in need of improvement. Under the Family Educational Rights and Privacy Act (FERPA), you must first provide your consent before DCPS, the Office of the State Superintendent (OSSE), or your child’s school can share your child’s education records with our organization.

I (parent/guardian name) _____________________hereby authorize and consent DCPS Office of the Chief of Staff, Office of the State Superintendent, or _______________________ (Name of School) to provide information concerning the education of my child, _________________________ (Name of Child), to After School All Stars and the DC Office of Out of School Time Grants and Youth Outcomes (OST Office). I further authorize the release of educational records of my child for the current school year to the parties listed above that include the following information: education transcripts, school/program enrollment information, universal student ID, address, demographic data, attendance data, credit history, grades, assessment data, IEP information, and graduation attainment (12th grade only). This authorization and release shall remain in effect for one calendar year from the date of signature.

By signing below, 1) I acknowledge and understand that I have the opportunity to review the records to be disclosed and the right to challenge the contents of such records, and 2) I am at least 18 years of age; and 3) I am signing this document on behalf of my child because he/she is not 18 years of age.

I, the Parent/Guardian hereby grant permission for my student(s) to attend a series of field trips during their time with ASAS.  The start and end times will be shared at least a week in advance. Possible field trip locations include Swimming Pool/Beach visits, National Museum of African American History, Museum of Illusions, The Spy Museum, The Movie Theater, and The Skating Rink. Both of us understand and expressly assume all of the risks and hazards associated with their participation.

I grant permission for my student(s) to [WALK/TAKE AN UBER/TAKE A BUS] from [School] to [TBD] under the direct supervision of an ASAS DC staff member.

I grant permission for ASAS DC to photograph and/or record my student(s) while participating in this activity, grant permission for publication of those images for any purpose, and hereby release any and all rights and interest which we may otherwise have in such photographs and/or video.

In consideration of my child’s participation in the activity, I hereby discharge and hold harmless After-School All-Stars DC and its directors, affiliates, managers, volunteers, employees, administrators, and assigns from any and all manners of action, rights of action, causes of action, suits, debts, accounts, damages, claims, liabilities, and demands whatsoever which I or our legal representative may have or may acquire against After-School All-Stars DC or its directors, affiliates, managers, volunteers, employees, administrators, or representatives, arising out of or in any way connected with my child’s participation in this activity. I agree After-School All-Stars DC has the right at its discretion to enforce established rules of conduct and/or terminate my child’s participation in the activity for failure to follow the rules of conduct, or for actions or conduct detrimental to or incompatible with the welfare, comfort, or interest of the activity as a whole.

STATEMENT OF COMMITMENT

In order for your child to be successful and receive the full benefits of participating in After-School All-Stars, it is important that each student attend daily. Attendance also plays a large part in how the program is funded. When students are absent, funding is reduced, increasing the possibility of the loss of the program.

• I understand that regular attendance is a requirement for participation in ASAS and that if my child’s attendance falls below 60% they will lose their spot in the program and will have to re-apply.
• I also understand that parental involvement is a vital component to the success of my child’s development and education. Therefore, as a parent/guardian of a student in the After-School All-Stars Program I will commit to the following:
  • I will make sure my child regularly attends the program.
  • I will respond in a timely manner to communications from ASAS staff
  • I will attend a minimum of 2 Family Engagement Activities hosted by ASAS
  • I will complete periodic surveys to assess the effectiveness of the program.

INTELLECTUAL PROPERTY RIGHTS SERVICE CONTENT, SOFTWARE AND TRADEMARKS

You acknowledge and agree that the After-School All-Stars virtual learning platform may contain content or features (“Online Content”) that are protected by copyright, patent, trademark, trade secret or other proprietary rights and laws. Except as expressly authorized by After School All-Stars, you agree not to modify, copy, frame, scrape, rent, lease, loan, sell, distribute or create derivative works based on the Online Content, in whole or in part, except that the foregoing does not apply to your own User Content (as defined below) that you legally upload to the After-School All-Stars virtual learning platform. For the sake of clarity, you acknowledge and agree that Class Recordings constitute Online Content belonging to After-School All-Stars. In connection with your use of the Online Content, you will not engage in or use any data mining, robots, scraping or similar data gathering or extraction methods. If you are blocked by After-School All-Stars from accessing the Online Content (including by blocking your IP address), you agree not to implement any measures to circumvent such blocking (e.g., by masking your IP address or using a proxy IP address). Any use of the Service or the Service Content other than as specifically authorized herein is strictly prohibited.

The After-School All-Stars name and logos are trademarks and service marks of After-School All-Stars (collectively the “After-School All-Stars Trademarks”). Other After School All-Stars, product, and service names and logos used and displayed via the Service may be trademarks or service marks of their respective owners who may or may not endorse or be affiliated with or connected to After-School All-Stars. Nothing in this Terms of Service or the Service should be construed as granting, by implication, estoppel, or otherwise, any license or right to use any of After-School All-Stars Trademarks displayed on the Service, without our prior written permission in each instance. All goodwill generated from the use of After-School All-Stars Trademarks will inure to our exclusive benefit.
