Risk Warning and Media Consent
I agree for my child to attend the program and to undertake all activities and/or to participate in the above program. In the case of an emergency, I authorise ASA Football where it is impracticable to communicate with me, to arrange for my child to receive such medical or surgical treatment as may be deemed necessary. I also undertake to pay or reimburse costs which may be incurred for medical attention, ambulance transport and drugs while my child is attending the program.I understand that although ASA Football and its service providers attempt to remove and minimise any risk of personal injury within practical boundaries, accidents do happen and all physical activities carry the risk of personal injury. I acknowledge that there is an inherent risk of personal injury in physical activities that will be undertaken in the program and I accept that risk.

To allow ASA football to use photographs, sound and film recordings taken of my child at this program only for the promotion of ASA Football services and initiatives to the media and to be used, reproduced, published and communicated to the public in any format or medium (including but not limited to print, digital, and online platforms) for the purposes described above, in Australia and overseas. You acknowledge that no payment or other compensation will be provided in connection with the use of the Media. You are aware that you may withdraw this consent at any time by providing written notice ASA Football.
Media Consent
Cancellation

Making payment shows your consideration and acceptance to ASA Football. You are aware that we can only offer a limited number of positions and the position once accepted is no longer free or available to another player. We disclosure that the payment of fees include but are not limited to cover expenses relating to all administration fees, team allocation and position, player information management, pre-order of uniforms and kits, registration of allocated teams for tournaments (Note the individual payment of child tournament registrations are separate) all training, training equipment, insurance and coaching fees. You acknowledge that all payments made are specifically allocated to cover all current and future expenses incurred by ASA Football specifically allocated to your child’s position. Your payment will remain to our discretion as non-refundable as it is proportionally allocated towards all expenses incurred. We reserve all rights to mitigating all financial risk caused to ASA Football as a result of any cancellation throughout the program. You acknowledge that any cancellation limits our position to the selection of another player. Such rights reserved shall include but not be limited to the retention and or recovery of the full payment of total registration fees as they are representative to the proportionate loss of expense and time incurred by ASA Football.
