Redford Leisure Services Activity/Event Waiver, Release, and Assumption of Risk Form Charter Township of Redford

I hereby voluntarily waive, release, and discharge to the fullest extent allowed by law, any and all actions, causes of action, or claims for personal injury, death or property damage which may result during my participation and/or use of any and all equipment in any activity/event sponsored by the Charter Township of Redford. This waiver discharges the Charter Township of Redford, their respective agents and employees from and against any and all liability arising out of, or connected in any way to, the participation in the activities/events and use of the facility. I assume full responsibility for and risk of personal injury, death, and property damage resulting from my participation in any activity or event. I further release and hold harmless the above mentioned entities and persons listed above. I also understand and agree that the waiver, release, and assumption of risk is binding to my heirs and assigns.

Photo/Video Authorization: I hereby give my consent for the Charter Township of Redford to use photos/video coverage of myself and/or minor child participating in a Charter Township of Redford sponsored program or event in future marketing materials. I acknowledge those pictured will only be named with their consent, or parent/guardian consent if younger than 18 years of age. I acknowledge event attendees must contact the photographer at the event if they wish themselves/their children to not be photographed.

Participant must check in at the main desk upon arriving at the facility. All event guests must have a wristband that must be worn at all times and cannot be transferred.

I/we agree to comply with all the Recreation & Wellness Center policies and procedures and understand that failure to comply may result in suspension or termination and any additional legal consequences depending on the severity of the incident. I further understand that I can find all policies posted on the Township website or at the main desk of the Recreation & Wellness Center.
