LIABILITY RELEASE: I certify that the information provided on this form is accurate and complete. I understand the nature of eyelash extension services and acknowledge that possible risks include, but are not limited to, irritation, redness, allergic reaction, sensitivity, discomfort, watering eyes, and premature lash shedding. I voluntarily consent to receive lash extension services and release the lash artist from liability arising from normal risks associated with the procedure, except in cases of gross negligence or misconduct.

1. I understand this lash extension service is being performed by a beginner lash artist who is currently improving speed and technique.
2. I understand appointment times may take significantly longer than a standard lash appointment and may exceed 5-6 hours.
3. I understand I must arrive with a completely open schedule and allow sufficient time for completion of the service.
4. I understand results may not be identical to inspiration photos and that lash styling may be adjusted based on my natural lashes.
5. I understand discounted pricing reflects the educational nature of this service, extended appointment time, and practice experience.

9. I give the artist permission for before-and-after photos and videos to be taken and used for educational, portfolio, marketing, and social media purposes.
Cancellation Policies
1. Cancellations with less than 24 hours' notice and no-shows will result in NO refunds.
2. No call/no shows forfeit your ability to book future appointments indefinitely.
3. I understand arriving more than 30 minutes late will result in a cancellation & rescheduling my appointment.
4. My appointment time is reserved specifically for me.
