Dear Parents/Guardians,

We are asking that you fill out this form if your child/children will not ride the bus after school. New this year we are asking you to select whether they will be picked up by you or if they will be staying after school for an extra curricular activity. We ask that you fill out one form per student. AM drop off changes do not need to be submitted, PM pickup changes need to be submitted no later than 11:30 am the day you will be picking them up. THE LESS CHANGES THE BETTER!
