Medical Release:  If we are unable to reach a parent or legal guardian (those you indicated on this form as Emergency Contacts), permission is granted to seek medical attention if necessary.

Media Release:  Permission is granted for your child's picture to be used in parish and Archdiocese publications, for example the weekly bulletin.

Parent Acknowledgment

I understand that I am registering my child(ren) for Religious Education classes at St. Marguerite Catholic Church. Classes will be held on Sundays according to the 2026–2027 Religious Education Calendar available on the parish website.

I understand that this registration is for Religious Education only and is not registration for First Holy Communion preparation. Participation in Religious Education is required as part of a child's preparation for any sacrament; however, First Holy Communion requires a separate registration process.

I understand that all children of St. Marguerite Catholic Church are expected to participate regularly in both Sunday Mass and Religious Education classes as part of their ongoing faith formation.
