- Firefighter

- 911 Operator
If you would like to nominate someone—or yourself—please complete this form in its entirety.
Submission Guidelines:
- Only one submission will be accepted per nominee.
- Do not encourage multiple people to submit nominations for the same individual. Only the first submission received will be considered, and additional submissions will not increase a nominee's chances of selection.
- Multiple submissions for the same nominee may result in disqualification of the nomination.
- Please review your answers carefully. Information submitted through this form may be published exactly as provided if the nominee is selected.
Selection Information:
- Submission does not guarantee selection.
- Selected honorees may be featured in print, on the URBANE Magazine website, on social media platforms, or any combination thereof.
- Not all selected honorees will appear in the print magazine.
The submission deadline is July 15, 2026.
The person completing this form will be notified shortly after the deadline if the nomination is selected. Nominees will not be contacted directly by URBANE Magazine. Once notified, you are encouraged to share the news with your nominee.
Please provide an email address that you check regularly, as email will be the primary method of communication regarding your submission.
