• HUB Kids 1 - 3 Registration Form

    This form is ONLY for grades 1 - 3. If you would like to register your child in grades 4 - 8, please visit wyandotconnections.org and choose that form.

  • 1st - 3rd grades will be held at the John Stewart Methodist Church (130 W Johnson Street, Upper Sandusky).

    Daily space is LIMITED, please choose only the days your child will need to come to HUB Kids after school.  Due to limited space in the program, regular attendance is expected. If a child is consistently absent for an extended period without communication from a parent or guardian, their spot may be offered to a child on the waiting list. Families wishing to return to the program will be placed back on the waiting list and accommodated as space becomes available.

  •  Sign-in and Sign-out

    Your child will be checked in when entering HUB Kids and checked out when leaving. (Children attending HUB Kids 1 - 3 at John Stewart will need to be signed out by an adult.) Once your child leaves HUB Kids, he/she will not be able to return that same day. 

    Illness

    Please do not send your child to HUB Kids if he/she is ill. If your child does not attend school due to illness they may not come to HUB Kids. They are welcome back when symptom free for 24 hours. If your child becomes ill while attending HUB Kids you will receive a call to pick up your child.

  • I, the parent/guardian of the student named above, agree with the following statements and I give permission for my child to participate in the following supervised activities organized by The HUB staff and volunteers:

    • I hereby give permission for my child to be transported from Union School to John Stewart for HUB Kids 1 - 3.
    • I agree that in the event of an emergency, injury, or illness during the drive, the staff member(s) in charge of the trip may act on my behalf and at my expense in obtaining medical treatment for my child.
    • Occasional Van Travel- Travel between John Stewart and The HUB. Notice will be provided to parents at least 48 hours in advance.
    • Supervised Walks- Walks around the block and in the surrounding area.
    • I acknowledge that when the group is away from John Stewart, a notice will be posted on the door indicating the group’s location. Parents or guardians may call HUB staff at any time to inquire about their child’s whereabouts or to arrange for pick-up. I understand that all activities will be properly supervised by HUB staff and volunteers. I authorize my child to participate in these outings under the conditions described above.
  • I (parent/legal guardian) give permission and consent to Wyandot Community Connections/The HUB/HUB Kids and third parties collaborating with The HUB/HUB Kids/WCC to make, reproduce, edit, broadcast, or rebroadcast any photo, video, soundtrack recording of my child (named above) while participating in The HUB/HUB Kids/WCC activities and/or programs, and including any written or spoken narrative account of experiences at The HUB/John Stewart for publication, display, or exhibition in promotions, advertising, and legitimate business uses by and for The HUB/HUB Kids/WCC. I further understand there will be no compensation to, and/or claim, by me or my child for such items.*
  • In the event that I cannot be reached in an emergency, I hereby give permission to medical personnel to order x-rays, routine tests, and treatment for my child if he or she becomes ill. I hereby give permission to the physician to hospitalize, secure proper treatment for, and order injection and/or anesthesia and/or surgery for my child as named here. I hereby give permission for my child to participate in screenings and testings, which may identify additional services which may benefit my child. I further agree to be financially responsible for all such medical services.*
  • I hereby give permission to The HUB/HUB Kids/Wyandot Community Connections and area community schools to exchange information regarding the minor child listed on this application. The purpose of the exchange is to help both organizations better prepare the student to be successful in life.*

  • By signing below:

    I, the parent or legal guardian, give my permission for the above-named individual to be transported (via WyandotCARES van) driven by a WCC/HUB qualified staff/volunteer to the HUB Kids at John Stewart Methodist Church.  

    I understand that all applicable laws regarding riding in a motor vehicle and directions provided by the driver and/or other adult volunteers must be followed.

    I, the parent or legal guardian, release Wyandot Community Connections, Inc (WCC) and WyandotCARES with all representatives, from any and all claims resulting from injury or damage that may be sustained by my child while participating in travel to and from the Union School and HUB Kids 1 - 3 at John Stewart Methodist Church (a program of Wyandot Community Connections).

    I, the parent or legal guardian, reviewed the Parent/Youth Handbook (download the handbook at wyandotconnections.org) with my child (named above). By signing below, we (parent and child) understand and agree to abide by the rules and regulations of The HUB/Wyandot Community Connections (“WCC”).

    I, the parent or legal guardian, agree and affirm as follows: in consideration for my child, as named here, participating in The HUB/HUB Kids/WCC programs and activities, I agree to assume the risk of such activities and further hold harmless The HUB/HUB Kids/WCC/John Stewart Methodist Church, employees, and volunteers from any and all claims, suits, losses, or related causes of action for damages, including but not limited to, such claims that may result from my/our injury or death, accidental or otherwise, during or arising in any way from my/our The HUB/HUB Kids/WCC/John Stewart Methodist Church involvement. I further represent and affirm that I have the legal capacity to bind any minor and or the heirs and or estate of same for whom this consent is given.

    I, the parent or legal guardian, acknowledge that The HUB/HUB Kids/Wyandot Community Connections is a faith-based organization, founded on the teachings of Jesus. Our After School program is open to all eligible youth, regardless of religious affiliation or belief.

    The information I have provided is current and correct to the best of my knowledge. I will let WCC know of any change in the information I have provided.
