• Required Tutorial for Parent Portal

  • Dear Parents and Guardians,

    Our Parent Portal the only way to communicate with our staff so that we can focus on creating the best educational experience for your child. This tutorial will assist you in navigating our system so that you can get your questions answered and feel heard. This is a mandatory tutorial. Thank you! 

  • As the parent/guardian for the above listed student, I understand that I will contact Faith Holistic Academy staff through the Parent Portal at https://faithholisticacademy.org/parent-portal/. If I am not sure what form to complete, I will complete a Student Inquiry Form to request guidance. I will not email Faith Holistic Academy staff directly and understand that if I do not communicate via the ParentPortal, Faith Holistic Academy staff are not required to respond to my email. Additionally, Faith Holistic Academy staff have 5 business days to respond, noting when our school is closed for a break, holiday or work day, we are not required to respond during this time. Please initial below that you understand our requirements, as stated in our Parent/Student Handbook.*
  • As the parent/guardian for the above listed student, I understand that the ABSENCE/TARDY Reporting Form is the best way to report absences and tardies. I will read the ABSENCE/TARDY Policy and the policies pertaining to absences and tardies. Please initial below that you understand this and will follow our Absence/Tardy Policy as found in our Parent/Student Handbook. *
  • As the parent/guardian for the above listed student, I understand that the CREDIT AUTHORIZATION form is generally completed during the month of July. For the 26-27 Instructionall Year, all parents must complete this form but moving forward, if you credit card on file does not change, then we will continue to use the same credit card authorization form.*
  • As the parent/guardian for the above listed student, I understand that the DONATION form is used to donate for parties, staff appreciation, etc. Please initial below that you understand this. *
  • As the parent/guardian for the above listed student, I understand that the Faith Holistic Academy Instructional Calendar is available on the Parent Portal and this will be the most updated copy available. I furthermore understand that Faith Holistic Academy does not follow the Marion County Public Schools calendar and my child is expected at school during days listed on the Faith Holistic Academy instructional calendar. Please initial below that you understand and will abide by this. *
  • As the parent/guardian for the above listed student, I understand that the MEDICATION DISPENSING FORM must be completed prior to dispensing medication and that we must have any prescribed medication in the original bottle or box with my child's name on it and must not be expired. OTC medications or supplements do not need a prescription unless federal or state law requires it. Please initial below that you understand this. Since our school name has changed, a new form or forms must be completed for medication to be dispensed.*
  • As the parent/guardian for the above listed student, I understand that the PAYMENTS Form (out of uniform, uniform order form, supply fee, registration fee) is a form that can be used to make payments for the items listed only. Of special note, you must pay IN ADVANCE for out of uniform Fridays. If you do not pay in advance, and your child shows up out of uniform on Friday, you will be billed for the entire quarter-no exceptions. Please initial below that you understand this. *
  • As the parent/guardian for the above listed student, I understand that the Unresloved Issues? AFTER FOLLOWING STEPS 1-4, COMPLETE THIS FORM is to be used only after following our Parent Concerns policy (as shown on the STUDENT INQUIRY form and in our Parent/Student Handbook). Please do not use this form unless you have exhausted all other options. This form goes directly to our board of directors so we ask that you follow our policies and exhaust all other options first. Please initial below that you understand this. *
  • Promptness and consistency are very important in a student's life. Please note that we are starting school 10 minutes earlier for the 25-26 Instructional Year and ending school 10 minutes earlier as well. All students should arrive no later than 8:34am, instruction starts promptly at 8:35am (after our morning prayer). Our front desk does not open until 8:25am. This time is based on our reception area clock. Students are considered tardy at 8:35am and at that time, must be signed in. When you are dropping off your child, you must park your vehicle in our parking lot (not Globalwide Realty’s parking lot) or in the overflow parking area (the grassy area near Capstone Academy) you must bring your child to one of our staff members-no exceptions. Please do not block the road at any time or drop your child off and leave him/her to walk on their own-this is strictly prohibited. The road in front of our building is a right of way to other buildings, so we must not block it. Late arrival is extremely disruptive to the teachers and students in a class so please arrive on time. Of final note, the instructional day ends at 2:35pm. You must pick up your child(ren) by no later than 2:44pm. Starting at 2:45pm-you will be charged $5.00 per minute. No exceptions. If your child is picked up late habitually, this may be cause for unenrollment or a required referral to Ong's Taekwondo.   • POLICY REMINDER (partial policy shown-see Parent/Student Handbook for full policy): Please immediately let us know if your child has received any LIVE VIRUS immunization or vaccine so that we can be protective of students or staff members that have medical conditions which may cause them to have issues. We kindly request that you make an appointment on a Friday, when possible, for your child to receive vaccines or immunizations. Your child will have their absence excused. Thank you for being respectful of this policy. (Of Special Note: We accept all vaccine exemptions & would be happy to assist you with information on this exemption. The State of FL does not require any student to receive vaccines & as a religious entity, we respect parents’ God given choice not to vaccine their child.) Please initial you understand and will alert us if your child has received a vaccine or immunization and do your best to make appointments on Friday if your child receives a live vaccine or immunization. *
  • POLICY REMINDER-HEALTHY HABITS-NUTRITION We incorporate nutrition and wellness in every aspect of school life. Due to this, parents ofour students (& prospective students) must ensure the following:-Healthy foods for snacks and lunch are packed and sent in to ensure that their child(ren) is prepared to learn.Healthy foods include leftovers from dinner (not fast food), sandwiches, chips, fruit, vegetables. Water is theonly drink that our students can consume at school (unless there is a medical reason, and we must have anote from a medical provider). Do not send microwave popcorn unless it is already popped.-We are a nut free school, so please make sure that no nuts are sent in or foods that are processed infacility that processes nuts to ensure that our students that have anaphylactic nut allergies are kept safe.-We do not allow sugary foods/drinks, only water (unless there is a medical reason, and we must have a notefrom a medical provider).Additionally, do not send your child to school with candy, desserts, cookies, donuts, etc.) fast food, and weprefer no food coloring in food or snacks. It is at our discretion to dispose of any food, snacks or drink broughtin to school that is in violation of this policy. Please ask questions if you are unsure about what to pack yourchild for lunch or snacks.Please pack your students’ lunch and snacks in an insulated lunch bag (middle & high school students mustbring a clear lunch bag and use an ice pack specifically for a lunch bag if food needs to be kept cold).Our staff will assist in heating up food for our K-2nd grade students, and all other students will have access tomicrowaves to heat their food. Glassware is permitted to heat/reheat food. Each class will have two snackperiods per day (if your child has a medical need, please provide proper medicaldocumentation).NOTATING AGAIN: Candy, sugary treats, soda, and sugary drinks of any kind are NOT allowed. Water isthe only beverage allowed for students unless a note from a medical provider stating medical necessityis provided. Otherwise, please do not send your child to school with any sugary drinks of any kind.Water bottle: Each student is required to bring a water bottle (closed top, glass, or stainless steelpreferred) each day. If your student's lunch is left at home, parents/guardians will be notified to bring a healthy,nutritious lunch (NO FAST FOOD ALLOWED, sub or regular sandwiches are permitted) and a bottle of waterto our front desk by 11am. Each student is required to bring a water bottle (closed top, glass, or stainless steel preferred) each day. Please initial that you understand and will follow this policy.*
  • BIRTHDAYS/OTHER CELEBRATIONS/RELIGIOUS OBSERVATION REGARDING HOLIDAYS/CELEBRATIONS Do not send any food or drink for a birthday or other celebrations without prior approval from our associate or our executive director (many of our students have food allergies). If approval is given to bring in a birthday snack (we generally limit snacks to a fruit platter with dipping sauce that is gluten free and free of dyes), we will only allow this during the last period of the instructional day-no exceptions. Of additional note, if your family does not celebrate birthdays or other celebrations due to religious observation, we must have a letter from a clergy member explaining what the child is not allowed to celebrate to make accommodations, if possible (or excuse the absence, if needed). Please initial you will follow this policy and provide any documentation from a clergy member, as required. *
