In the unlikely event there is a problem at the hospital or a medical or behavioral issue with your pet, we will attempt to contact you first. However, if we are unable to reach you or need someone local to pick up your pet, who can we contact? Be sure to let them know we may reach out and they will be expected to care for your pet if we are unable.

Are vaccines current? (Vaccines must be updated a minimum of 2 weeks before check in to allow for sufficient immuno-response.)
Would you like a bath? Extra charges apply. (Bathing is only available for dogs.)
Would you like a nail trim? Extra charges apply.
Should we feed our food (GI Safe Diets) or your food? Your food is recommended to prevent tummy troubles.

Medications: All medications must be labeled and in their original container. Please list all medications your pet is on and the dose and frequency for each.

All pets boarding with us at APH are required to be current on vaccinations and flea/tick preventatives. Any pets overdue will be treated upon admission at owner's expense.

I have received a copy of the Alameda Pet Hospital Boarding Policy and Boarding Waiver and Release of Liability. I understand and agree to their terms and conditions. I understand I will need to review and sign these documents every 6 months to continue using the boarding facilities at Alameda Pet Hospital.
