THIS AGREEMENT is entered into by and between Alameda Pet Hospital (APH) and the pet owner listed above with the following terms:

2. Owner understands that APH has relied upon their representation that their dog is in good health at the time of check-in for services at our facility. Any pre-existing health concerns or known allergies will be disclosed and discussed prior to check-in. Furthermore, owner understands that we do not have a licensed veterinarian on site 24 hours per day and our kennel staff is not medically trained to detect or diagnose a medical condition in your pet. Any medical care is solely at the discretion of the veterinarian in charge.3. Owner specifically represents to APH that to their knowledge their pet has not been exposed to any contagious diseases within a thirty-day period prior to check-in for boarding at our facility. Owner further agrees to maintain required current vaccinations, and provide proof when requested, for their pet(s). Required canine vaccines: Rabies, Distemper, Bordetella, Canine Influenza. Required feline vaccines: Rabies, FVRCP.4. Owner understands that if their pet becomes ill or injured, or if the state of the pet's health otherwise requires professional attention, APH, in a non-emergency situation, will attempt to contact and inform the owner prior to providing medical care. If no contact can be made, or in an emergency situation, APH at its sole discretion, may provide reasonable veterinary care and thereafter attempt contact with the owner. Furthermore, all expenses incurred from this treatment will be the sole responsibility of the owner.5. Owner understands that senior pets, or any animal with a chronic medical condition, are required to be cleared by an APH veterinarian to ensure the pet is stable and free of health concerns that would present a risk while boarding in a non-medical kennel environment. Furthermore, the owner acknowledges the risks associated with allowing a senior pet to participate in boarding or daycare services, even with the written consent of a veterinarian.6. APH and its employees shall exercise reasonable care for pets under our supervision. While animals are only allowed group play with members of their household, the yard is a shared space that may be contaminated with ill patients. Every attempt is made to thoroughly clean between exposure and prevent pathogen transmission. Owner understands that sick animals come to the hospital and their pet(s) may be exposed to serious diseases.12. Owner understands that if their pet is not picked up on or before the scheduled check-out date, they hereby authorize APH to take whatever action is deemed necessary for continued care of their animal. Owner will be responsible for the cost of any such continuing care upon demand by APH and understands that if they do not pick up their pet, APH will proceed after 5 days of no communication or payment with arrangement according to the guidelines provided by the (California State Abandonment Animal Statute) Abandonment of animals by owner; procedure for handling. Owner also acknowledge that they will be fully responsible for all attorney's fees and associated costs if they abandon their pet.14. Owner further understands and expressly agrees that each and every of the foregoing provisions contained above shall be in force and effect and shall apply to each and every occasion on which the owner's animal(s) stay(s) with APH for daycare, extended boarding, or any other kennel or grooming services, as the case may be. This Agreement shall remain in full force and effect as between the parties for a period of 6 months unless otherwise cancelled or suspended by a writing signed by the parties.

11. I understand that by allowing their pet to participate in services offered by APH, they hereby agree to allow APH to take photographs or use images of their pet in print or otherwise for publication, promotion, or social media.

Owner hereby certifies that Owner has read and understands this Waiver and Release of Liability and the regulations set forth above. By signing this agreement, Owner agrees to be bound by its terms and conditions.
