I hereby give permission to Traeh Thyssen Have a hearT Foundation to use my photos and likeness in all forms and media to continue to reach the foundation's mission and vision to bring awareness to and prevent bullying and youth suicide and all other lawful purposes. 
I understand I am entitled to no compensation. I release the photographer of all forms of claims and liability related to my photo usage. I am over 18 years of age or I am the parent/guardian of the minor listed above, and I fully understand the terms of this release.
