    Thank you so much for popping in here for a quick questionnaire for an investigative reporting piece that covers trans healthcare access in Kentucky.
  • My name is Mo Viviane (they/she/he/its), and I'm an Independent Journalist from Rural Kentucky, who reports on the experiences of LGBTQIA+ Kentuckians. I've been featured in Queer Kentucky, Prism Reports, and Black Youth Project, covering topics around harm reduction, identity, spirituality, and LGBTQIA+ artists.

    The questions in this form are for an investigative article on healthcare access, incarceration, and the policies that affect these intersections. All of this includes the discussion of the "One Big Beautiful Bill," and section 7301 of title 5 United States Code, and their effects on Rural Trans Kentuckians, with priority to Black and Brown narratives.

    Through these questions, please only share what you feel comfortable sharing. You can skip questions, and stop answering at any time. You are welcome to reach out to me directly (you can find my contact in the link on my name), if you would like to have a one-to-one interview instead.

    Your answers to these questions will only be shared with me (the reporter) and the editing team of Prism. These answers will be stored on my personal device.
