Acknowledgment of Risk: I, the undersigned participant (or parent/guardian if the participant is
under 18 years of age), understand that participating in activity at the Push Movement facility and
skatepark (located at 831 SE MLK Blvd., Portland, OR 97214) involves inherent risks, including but
not limited to falls, collisions, and other physical injuries. I acknowledge that such risks may result in
serious injury or death.

Community Guidelines: Push Movement is committed to creating a sober, safe, welcoming, and
inclusive environment for everyone. By participating in our programs and using our facilities, I agree
to treat others with respect, kindness, and consideration regardless of age, race, ethnicity, gender
identity, sexual orientation, ability, religion, or background. I will help create a positive community
by encouraging others, resolving conflicts peacefully, using appropriate language, respecting personal
boundaries, and caring for shared spaces and equipment. I understand that bullying, harassment,
discrimination, violence, substance use, vandalism, or behavior that threatens the safety or well-being
of others is not permitted. I acknowledge that participation in Push Movement programs is a shared
responsibility, and I will contribute to a culture of respect, accountability, and belonging for all
members of the community.

Hold Harmless Agreement: In consideration of my (or my child's) participation at the Push
Movement facility and skatepark, I hereby agree to hold harmless and indemnify Push Movement, its
board members, facility owners, employees, agents, and volunteers from any and all claims,
demands, actions, causes of action, liabilities, damages, losses, or expenses arising out of or in any
way related to my (or my child's) participation, including but not limited to any transportation
provided by Push Movement.
This agreement applies to any injury or damage resulting from negligence or other actions taken by

Push Movement or its representatives in connection with my (or my child's) participation. I understand that this hold harmless agreement is intended to protect Push Movement from any liability associated with skateboarding or other activities in the facility and that I have carefully read and understand this provision.

In consideration of being allowed to participate in skateboarding or other activities held at the Push Movement facility and the transportation provided by Push Movement, I hereby agree to the following:

1. Release: I hereby release, waive, and discharge Push Movement, its directors, officers, building owners, employees, volunteers, agents, and affiliates (collectively referred to as "Releasees") from any and all claims, liabilities, or demands arising out of or related to my participation at the Push Movement facility and transportation provided by Push Movement.

2. Indemnification: I agree to indemnify and hold harmless Releasees from any loss, liability, damage, or costs incurred due to my participation at the Push Movement facility or related to transportation provided by Push Movement.

3. Medical Treatment Authorization: In the event of an emergency or medical situation while participating in activities with Push Movement, I authorize Push Movement representatives to seek medical treatment for me (or my child) as deemed necessary.

4. Photography and Media Release: I consent to the use of photographs or videos taken during program activities for promotional purposes by Push Movement. If I do not wish for my likeness or that of my child to be used, I will notify Push Movement in writing.

Acknowledgment of Understanding: I have read this waiver carefully and fully understand its contents. I acknowledge that by signing this document, I am waiving certain legal rights that may be available to me. If I am signing on behalf of a minor child, I affirm that I am the parent or legal guardian of said child and have full authority to execute this waiver on their behalf.
